La complejidad del espectro del dolor orofacial se ve agravada por la proximidad
de numerosas estructuras anatómicas, que incluyen, ojos, nariz, dientes, lengua,
senos paranasales, oídos, musculatura regional y la articulación
temporomandíbular. Estas estructuras pueden ser el origen del dolor facial que
puede referir a áreas cercanas o próximas pero que no estén afectadas. Por
ejemplo, no es raro que existan referencias cruzadas entre las cefaleas y otras
condiciones de dolor orofacial.
Existen numerosos paradigmas para la evaluación y tratamiento eficaz o exitoso
del dolor orofacial. Históricamente, el dolor orofacial ha sido catalogado o
categorizado en cuatro grupos basados en los mecanismos del dolor – llamados:
musculoesquelético, neuropático, neurovascular y con implicación psicológica
(psicógeno). Otro paradigma más mecanicista sugiere que el dolor orofacial
puede ser de causa nociceptiva, inflamatoria, neuropática o puede que el dolor
no obedezca a daño alguno de los tejidos. La fichas descriptivas del dolor
orofacial, originariamente desarrolladas durante el Año Global contra el dolor
orofacial siguen estos paradigmas.
