(*) Required information.

(1) The numbers of the registration form have to be correlatives.
(2) It is allowed, for the Spanish citizens, the ID document, passport, and driving licence; for the foreigners, the passport, the letter or ID document of citizens from
European Union, Andorra, Iceland, Switzerland, Norway, Malta, Monaco and San Marino. Passport for the rest of foreigners. Spanish residence permit in force for the
foreigners resident in Spain. Residence permit sent by another member state of the European Union. You have to fill: D = DNI (ID document), P = Passport, C = Driving
licence, I = Letter or ID document, N = Spanish residence permit, X = Residence permit of another member state of the European Union.
(3) F = feminine sex, and M = masculine sex.
(4) You have to fill with the format YYYYMMDD. You have to fill at least the year of birth. The month and the day, if you do not know them, it will appear as 0000.
(5) Name of the country of nationality.
(6) You have to fill with the format YYYYMMDD.
