Hazard Identification and Risk Assessment
with the HIRARC at unit production of PAC 280 AC

1. Introduction
1.1 Purpose
The purpose of this report is to know the hazard level and OSH risk for the
corrective and maintenance measures with the HIRARC (Hazard Identification
Risk Assessment and Risk Control) method and to prevent and control workplace
accidents.

1.2 Background
For any industry to be successful it should meet not only the production
requirements, but also maintain the highest safety standards for all concerned.
The industry has to identify the hazards, assess the associated risks and bring the
risks to tolerable level on a continuous basis. Chemical industry being a
hazardous operation has considerable safety risk to workers. Unsafe conditions
and practices lead to a number of accidents and causes loss and injury to human
lives, damages the property, interrupt production etc. Risk assessment is a
systematic method of identifying and analyzing the hazards associated with an
activity and establishing a level of risk for each hazard. The hazards cannot be
completely eliminated, and thus there is a need to define and estimate an accident
risk level possible to be presented either in quantitative or qualitative way.
Hazard identification and risk assessment and control are part of the risk
management system which is the basis of the Occupational Safety and Health
Management System OHS system consisting of hazard identification, risk
assessment and risk control.

This report is an observational research. And primary data from survey
collect by inspecting the workplace, response of employees regarding possible
hazard in their workplace. Secondary data comes from reviewing department
procedure manual, work instruction, standard operating procedure, previous
incident reports, safety data sheet, first aid/ injury register, and health record of
employee.

1.4 Scope
Based on the identification of the above, this report looks in to the risk
assessment on the production unit of PAC 280 AC at PT Pacinesia Chemical
Industry with the HIRARC (Hazard Identification Risk Assessment and Risk
Control) method to find out the level of risk, and determine control measure.
