Note: TOC reserves the right to request for further medical documents in accordance to the declared health issue(s) before enrollment. Upon such request, you are to submit
the necessary document together with TOC’s medical report form and medical report.

c. Insurance

Malaysian students will be provided with an Insurance coverage by TOC for the entire tenure of their studies which is included in the miscellaneous fee
charged as per the fee structure. Termination of the policy must be done through written letter before Orientation and/or 1 (one) month before the end
of policy. The policy cycle is as per the intake enrolled. The coverage is bound to the terms and conditions set forth in the said policy and students need
to adhere to these terms and conditions.

Students should adhere to the terms and conditions of the policy and any additional cost incurred in respect to hospitalization will be borne by the
student/parent/guardian within 30 days from the date of the invoice by TOC failing which students can be barred from the terms examination.

Medical Declaration Form within 2 weeks from the date of LOCO.
e. Registrar Office will validate the Medical Declaration Form and issue the
Letter of Offer with the Acceptance Acknowledgement Form.
f. Student to accept the offer by paying the 1st term fee.
