Adalah sikap seseorang yang bernapas dengan mulut mencucu dan ekspirasi yang memanjang. Sikap ini terjadi
sebagai mekanisme tubuh untuk mengeluarkan retensi CO2 yang terjadi sebagai mekanisme tubuh untuk
mengeluarkan retensi CO2 yang terjadi pada gagal napas kronik.

Pursed-lip breathing enhances airflow because it increases the resistance to the outflow of air and helps to
prevent airway collapse by increasing airway pressure.
Persons with severe airflow obstruction may also exhibit use of the accessory muscles, often sitting in the
characteristic “tripod” position in which the arms are braced to facilitate use of the sternocleidomastoid,
scalene, and intercostal muscles.
With loss of lung elasticity and hyperinflation of the lungs, the airways often collapse during expiration because
pressure in surrounding lung tissues exceeds airway pressure. Air becomes trapped in the alveoli and lungs,
producing an increase in the anteroposterior dimensions of the chest, the so-called barrel chest that is typical
of persons with emphysema.

The classic presentation in emphysema with no “bronchitic” component is one in which the patient is barrel-
chested and dyspneic, with obviously prolonged expiration, sitting forward in a hunched-over position,
attempting to squeeze the air out of the lungs with each expiratory effort.

The inflammation induced by cigarette smoke is a trigger of the cycle of protease release and lung
destruction resulting in emphysema. Macrophages are activated by cigarette smoke and recruit neutrophils
and other inflammatory cells to the lung, leading to the release of elastase and MMPs. The destruction of
elastin and other connective tissue elements in the lungs over time by these proteases leads in turn to the loss
of elastic recoil and the destruction of alveolar structures characteristic of emphysema.
Cigarette smoke contains many oxidant molecules, capable of inducing oxidative stress in the lung.
Oxidative stress has diverse effects, including the oxidative inactivation of antiproteases in the lung as well as
acetylation of specific histones in the chromatin of lung cells and macrophages, allowing the expression of
various pro-inflammatory genes. Histone deacetylase activity is reduced in COPD, which in turn may result in
an inability to control the pro-inflammatory response in this condition. Pro-inflammatory gene expression
promotes cytokine production and release, contributing to further inflammatory cell recruitment and
activation.

Tar – this is the collective term for the various particles suspended in tobacco smoke. The particles
contain chemicals, including several cancer-causing substances (carcinogens). Tar is sticky and brown,
and stains teeth, fingernails and lung tissue. Tar contains the carcinogen benzo(a)pyrene.
Carbon monoxide – this odourless gas is fatal in large doses because it takes the place of oxygen in the
blood. Each red blood cell contains a protein called haemoglobin that transports oxygen molecules
around the body. However, carbon monoxide binds to haemoglobin better than oxygen. In response,
the body makes more red blood cells to carry the oxygen it needs, but it makes the blood thicker. This
means that when the body demands more oxygen during exercise, less oxygen reaches the brain,
heart, muscles and other organs.
Hydrogen cyanide – the lungs contain tiny hairs (cilia) that help to clean the lungs by moving foreign
substances out. Hydrogen cyanide stops this lung clearance system from working properly, which
means the poisonous chemicals in tobacco smoke can build up inside the lungs. Other chemicals in
smoke that damage the lungs include hydrocarbons, nitrous oxides, organic acids, phenols and
oxidising agents.
Oxidizing chemicals – these highly reactive chemicals (which include free radicals) can damage the
heart muscles and blood vessels. They react with cholesterol, leading to the build-up of fatty material
on artery walls. Their actions lead to heart disease, stroke and blood vessel disease.
Metals – tobacco smoke contains dangerous metals including arsenic, cadmium and lead. Several of
these metals are carcinogenic.
Radioactive compounds – tobacco smoke contains radioactive compounds that are known to be
carcinogenic.

9. Apa interpretasi dari hasil pemeriksaan pasien?
FEV/FCV < 70 %
Diffuse pulmonary diseases can be classified into two categories : (1) obstructive (airway) disease,
characterized by limitation of airflow, usually resulting from an increase in resistance caused by partial or
complete obstruction at any level, and (2) restrictive disease, characterized by reduced expansion of lung
parenchyma accompanied by decreased total lung capacity.
The major diffuse obstructive disorders are emphysema, chronic bronchitis, bronchiectasis, and
asthma. In patients with these diseases, forced vital capacity (FVC) is either normal or slightly decreased, while
the expiratory flow rate, usually measured as the forced expiratory volume at 1 second (FEV1), is significantly
decreased. Thus, the ratio of FEV to FVC is characteristically decreased.
By contrast, in diffuse restrictive diseases, FVC is reduced and the expiratory flow rate is normal or
reduced proportionately. Hence, the ratio of FEV to FVC is near normal.

An acinus (/ˈæsɨnɨs/) (plural, acini; adjective, acinar [/ˈæsɨnər/] or acinous) refers to any cluster of cells that
resembles a many-lobed "berry", such as a raspberry (acinus is Latin for "berry"). The berry-shaped
termination of an exocrine gland, where the secretion is produced, is acinar in form, as is the alveolar sac
containing multiple alveoli in the lungs.

12. Bagaimana penatalaksanaan dari kasus?
Penatalaksanaan secara umum PPOK meliputi :
1) Edukasi
Agar edukasi dapat diterima dengan mudah dan dapat dilaksanakan ditentukan skala priority bahan
edukasi sebagai berikut :
a. Berhenti merokok
Disampaikan pertama kali kepada penderita pada waktu diagnosis PPOK ditegakkan
b. Pengunaan obat - obatan
Macam obat dan jenisnya
Cara penggunaannya yang benar ( oral, MDI atau nebuliser )
Waktu penggunaan yang tepat ( rutin dengan selangwaku tertentu atau kalau perlu
saja )
Dosis obat yang tepat dan efek sampingnya
c. Penggunaan oksigen
Kapan oksigen harus digunakan
Berapa dosisnya
Mengetahui efek samping kelebihan dosis oksigen
d. Mengenal dan mengatasi efek samping obat atau terapi oksigen
e. Penilaian dini eksaserbasi akut dan pengelolaannya
Tanda eksaserbasi :
Batuk atau sesak bertambah
Sputum bertambah
Sputum berubah warna
f. Mendeteksi dan menghindari pencetus eksaserbasi
g. Menyesuaikan kebiasaan hidup dengan keterbatasan aktiviti
2) Obat – obatan
a. Bronkodilator
Diberikan secara tunggal atau kombinasi dari ketiga jenis bronkodilator dan disesuaikan dengan
klasifikasi derajat berat penyakit ( lihat tabel 2 ). Pemilihan bentuk obat diutamakan inhalasi,
nebuliser tidak dianjurkan pada penggunaan jangka panjang. Pada derajat berat diutamakan
pemberian obat lepas lambat ( slow release ) atau obat berefek panjang ( long acting ).
Macam - macam bronkodilator :
- Golongan antikolinergik
Digunakan pada derajat ringan sampai berat, disamping sebagai bronkodilator juga
mengurangi sekresi lendir ( maksimal 4 kali perhari ).
- Golongan agonis beta – 2
Bentuk inhaler digunakan untuk mengatasi sesak, peningkatan jumlah penggunaan dapat
sebagai monitor timbulnya eksaserbasi. Sebagai obat pemeliharaan sebaiknya digunakan
bentuk tablet yang berefek panjang. Bentuk nebuliser dapat digunakan untuk mengatasi
eksaserbasi akut, tidak dianjurkan untuk penggunaan jangka panjang. Bentuk injeksi
subkutan atau drip untuk mengatasi eksaserbasi berat.
- Kombinasi antikolinergik dan agonis beta – 2
Kombinasi kedua golongan obat ini akan memperkuat efek bronkodilatasi, karena
keduanya mempunyai tempat kerja yang berbeda. Disamping itu penggunaan obat
kombinasi lebih sederhana dan mempermudah penderita.
- Golongan xantin
Dalam bentuk lepas lambat sebagai pengobatan pemeliharaan jangka panjang, terutama
pada derajat sedang dan berat. Bentuk tablet biasa atau puyer untuk mengatasi sesak
(pelega napas), bentuk suntikan bolus atau drip untuk mengatasi eksaserbasi akut.
Penggunaan jangka panjang diperlukan pemeriksaan kadar aminofilin darah.
b. Antiinflamasi
Digunakan bila terjadi eksaserbasi akut dalam bentuk oral atau injeksi intravena, berfungsi
menekan inflamasi yang terjadi, dipilih golongan metilprednisolon atau prednison. Bentuk
inhalasi sebagai terapi jangka panjang diberikan bila terbukti uji kortikosteroid positif yaitu
terdapat perbaikan VEP1 pascabronkodilator meningkat > 20% dan minimal 250 mg.
c. Antibiotika
Hanya diberikan bila terdapat infeksi. Antibiotik yang digunakan :
- Lini I : amoksisilin
makrolid
- Lini II : amoksisilin dan asam klavulanat
Sefalosporin
Kuinolon
makrolid baru
Perawatan di Rumah Sakit dapat dipilih :
- Amoksilin dan klavulanat
- Sefalosporin generasi II & III injeksi
- Kuinolon per oral
ditambah dengan yang anti pseudomonas
- Aminoglikose per injeksi
- Kuinolon per injeksi
- Sefalosporin generasi IV per injeksi
d. Antioksidan
Dapat mengurangi eksaserbasi dan memperbaiki kualiti hidup, digunakan N - asetilsistein. Dapat
diberikan pada PPOK dengan eksaserbasi yang sering, tidak dianjurkan sebagai pemberian yang
rutin.
e. Mukolitik
Hanya diberikan terutama pada eksaserbasi akut karena akan mempercepat perbaikan
eksaserbasi, terutama pada bronkitis kronik dengan sputum yang viscous. Mengurangi
eksaserbasi pada PPOK bronkitis kronik, tetapi tidak dianjurkan sebagai pemberian rutin.
3) Terapi oksigen
Indikasi
- Pao2 < 60mmHg atau Sat O2 < 90%
- Pao2 diantara 55 - 59 mmHg atau Sat O2 > 89% disertai Kor Pulmonal, perubahan P pulmonal, Ht
>55% dan tanda - tanda gagal jantung kanan, sleep apnea, penyakit paru lain.
4) Ventilasi mekanik
Ventilasi mekanik dapat dilakukan dengan cara :
- ventilasi mekanik tanpa intubasi
Bentuk ventilasi mekanik tanpa intubasi adalah Nonivasive Intermitten Positif Pressure (NIPPV)
atau Negative Pessure Ventilation (NPV).
Indikasi penggunaan NIPPV
- Sesak napas sedang sampai berat dengan penggunaan muskulus respirasi dan abdominal
paradoksal
- Asidosis sedang sampai berat pH < 7,30 - 7, 35
- Frekuensi napas > 25 kali per menit
NPV tidak dianjurkan karena dapat menyebabkan obstruksi saluran napas atas, disamping harus
menggunakan perlengkapan yang tidak sederhana.
- ventilasi mekanik dengan intubasi
Indikasi penggunaan ventilasi mekanik invasif :
- Sesak napas berat dengan penggunaan muskulus respirasi tambahan dan pergerakan
abdominal paradoksal
- Frekuensi napas > 35 permenit
- Hipoksemia yang mengancam jiwa (Pao2 < 40 mmHg)
- Asidosis berat pH < 7,25 dan hiperkapni (Pao2 < 60 mmHg)
- Henti napas
- Somnolen, gangguan kesadaran
- Komplikasi kardiovaskuler (hipotensi, syok, gagal jantung)
- Komplikasi lain (gangguan metabolisme, sepsis, pneumonia, emboli paru, barotrauma,
efusi pleura masif)
- Telah gagal dalam penggunaan NIPPV
5) Nutrisi
Mengatasi malnutrisi dengan pemberian makanan yang agresis tidak akan mengatasi masalah, karena
gangguan ventilasi pada PPOK tidak dapat mengeluarkan CO2 yang terjadi akibat metabolism
karbohidrat. Diperlukan keseimbangan antara kalori yang masuk denagn kalori yang dibutuhkan, bila
perlu nutrisi dapat diberikan secara terus menerus (nocturnal feedings) dengan pipa nasogaster.
Komposisi nutrisi yang seimbang dapat berupa tinggi lemak rendah karbohidrat. Kebutuhan protein
seperti pada umumnya, protein dapat meningkatkan ventilasi semenit oxygen comsumption dan
respons ventilasi terhadap hipoksia dan hiperkapni. Tetapi pada PPOK dengan gagal napas kelebihan
pemasukan protein dapat menyebabkan kelelahan. Gangguan keseimbangan elektrolit sering terjadi
pada PPOK karena berkurangnya fungsi muskulus respirasi sebagai akibat sekunder dari gangguan
ventilasi. Gangguan ini dapat mengurangi fungsi diafragma. Dianjurkan pemberian nutrisi dengan
komposisi seimbang, yakni porsi kecil dengan waktu pemberian yang lebih sering.
6) Rehabilitasi
Program rehabilitiasi terdiri dari 3 komponen yaitu : latihan fisis, psikososial dan latihan pernapasan.
Latihan fisis bagi penderita PPOK dapat dilakukan di dua tempat :
• Di rumah
- Latihan dinamik
- Menggunakan otot secara ritmis, misal : jalan, joging, sepeda
• Rumah sakit
- Program latihan setiap harinya 15-30 menit selama 4-7 hari per minggu. Tipe latihan diubah
setiap hari. Pemeriksaan denyut nadi, lama latihan dan keluhan subyektif dicatat. Pernyataan
keberhasilan latihan oleh penderita lebih penting daripada hasil pemeriksaan subyektif atau
obyektif. Pemeriksaan ulang setelah 6-8 minggu di laboratorium dapat memberikan
informasi yang obyektif tentang beban latihan yang sudah dilaksanakan.
- Dua bentuk latihan dinamik yang tampaknya cocok untuk penderita di rumah adalah
ergometri dan walking-jogging. Ergometri lebih baik daripada walkingjogging. Begitu jenis
latihan sudah ditentukan, latihan dimulai selama 2-3 menit, yang cukup untuk menaikkan
denyut nadi sebesar 40% maksimal. Setelah itu dapat ditingkatkan sampai mencapai denyut
jantung 60%-70% maksimal selama 10 menit. Selanjutnya diikuti dengan 2-4 menit istirahat.
Setelah beberapa minggu latihan ditambah sampai 20-30 menit/hari selama 5 hari
perminggu. Denyut nadi maksimal adalah 220 - umur dalam tahun.
- Apabila petunjuk umum sudah dilaksanakan, risiko untuk penderita dapat diperkecil.
walaupun demikan latihan jasmani secara potensial akan dapat berakibat kelainan fatal,
dalam bentuk aritmia atau iskemi jantung.
Hal-hal yang perlu diperhatikan sebelum latihan :
- Tidak boleh makan 2-3 jam sebelum latihan
- Berhenti merokok 2-3 jam sebelum latihan
- Apabila selama latihan dijumpai angina, gangguan mental, gangguan koordinasi atau
pusing latihan segera dihentikan
- Pakaian longgar dan ringan

Right-sided heart failure. Severe hypoxemia, in which arterial PO2 levels fall below 55 mm Hg, causes
reflex vasoconstriction of the pulmonary vessels and further impairment of gas exchange in the lung.
It is more common in persons with the chronic bronchitis form of COPD. Hypoxemia also stimulates
red blood cell production, causing polycythemia. The increase in pulmonary vasoconstriction and
subsequent elevation in pulmonary artery pressure further increase the work of the right ventricle. As
a result, persons with COPD may develop right-sided heart failure with peripheral edema (i.e., cor
pulmonale).

16. Klasifikasi ppok
Penyakit Paru Obstruktif Kronik (PPOK)
PPOK adalah penyakit paru kronik yang ditandai oleh hambatan aliran udara di saluran napas yang bersifat
progressif nonreversibel atau reversibel parsial. PPOK terdiri dari bronkitis kronik dan emfisema atau
gabungan keduanya.
Bronkitis kronik
Kelainan saluran napas yang ditandai oleh batuk kronik berdahak minimal 3 bulan dalam setahun, sekurang-
kurangnya dua tahun berturut - turut, tidak disebabkan penyakit lainnya.
Emfisema
Suatu kelainan anatomis paru yang ditandai oleh pelebaran rongga udara distal bronkiolus terminal, disertai
kerusakan dinding alveoli.
Secara anatomik dibedakan tiga jenis emfisema:
Emfisema sentriasinar, dimulai dari bronkiolus respiratori dan meluas ke perifer, terutama mengenai
bagian atas paru sering akibat kebiasaan merokok lama
Emfisema panasinar (panlobuler), melibatkan seluruh alveoli secara merata dan terbanyak pada paru
bagian bawah
Emfisema asinar distal (paraseptal), lebih banyak mengenai saluran napas distal, duktus dan sakus
alveoler. Proses terlokalisir di septa atau dekat pleura

Types of Emphysema
Emphysema is classified according to its anatomic distribution within the lobule; as described earlier, the
acinus is the structure distal to terminal bronchioles, and a cluster of three to five acini is called a lobule. There
are four major types of emphysema: (1) centriacinar, (2) panacinar, (3) distal acinar, and (4) irregular. Only the
first two types cause clinically significant airway obstruction, with centriacinar emphysema being about 20
times more common than panacinar disease.
1) Centriacinar (Centrilobular) Emphysema
The distinctive feature of centriacinar (centrilobular) emphysema is the pattern of involvement of the
lobules: The central or proximal parts of the acini, formed by respiratory bronchioles, are affected,
while distal alveoli are spared. Thus, both emphysematous and normal air spaces exist within the same
acinus and lobule. The lesions are more common and severe in the upper lobes, particularly in the
apical segments. In severe centriacinar emphysema the distal acinus also becomes involved, and thus,
the differentiation from panacinar emphysema becomes difficult. This type of emphysema is most
commonly seen as a consequence of cigarette smoking in people who do not have congenital
deficiency of α1-antitrypsin.
2) Panacinar (Panlobular) Emphysema
In panacinar (panlobular) emphysema, the acini are uniformly enlarged, from the level of the
respiratory bronchiole to the terminal blind alveoli. In contrast with centriacinar emphysema,
panacinar emphysema tends to occur more commonly in the lower lung zones and is the type of
emphysema that occurs in α1-antitrypsin deficiency.
3) Distal Acinar (Paraseptal) Emphysema
In distal acinar (paraseptal) emphysema, the proximal portion of the acinus is normal but the distal
part is primarily involved. The emphysema is more striking adjacent to the pleura, along the lobular
connective tissue septa, and at the margins of the lobules. It occurs adjacent to areas of fibrosis,
scarring, or atelectasis and is usually more severe in the upper half of the lungs. The characteristic
finding is the presence of multiple, contiguous, enlarged air spaces ranging in diameter from less than
0.5 mm to more than 2.0 cm, sometimes forming cystic structures that, with progressive enlargement,
are referred to as bullae. The cause of this type of emphysema is unknown; it is seen most often in
cases of spontaneous pneumothorax in young adults.
4) Irregular Emphysema
Irregular emphysema, so named because the acinus is irregularly involved, is almost invariably
associated with scarring, such as that resulting from healed inflammatory diseases. Although clinically
asymptomatic, this may be the most common form of emphysema.
