1. Participants come to the Medical Check Up (MCU) on time according to the


schedule that has been made by the company / Department with the ID badge and
identity card (passport).
2. Listed participant from Department must come to clinic town site 1 to re register
his/her self on the scheduled date not more than 09.00 am, and participants have
to be patient to wait for their turn on each regular inspection.
3. Unlisted participant can only be served after 09.00 am when the listed participant
still not meets the quota yet.

4. After 09.00 am, both listed and unlisted participant allowed to register when the
MCU participant still not meets the quota (70 participants/day), if it meets the quota
the registration officer(s) will refuse the candidate whether he/she is listed by
Department.

5. The MCU schedule is:


5.1. Monday to Friday: from 07.30 WIB to 12.00 WIB then from 13:00 WIB to
14:00 WIB (break from 12.00 WIB to 13.00 WIB).
5.2. Saturday: from 7:30 WIB to 11:00 WIB.
6. Annual MCU Participants are RAPP’s employees that have worked at least 1 year
(Latest joint date on 31 May 2016).
7. Participants are required to fast for minimal 8 hours in advance (do not eat and
drink anything except drinking mineral water)
8. Participants MUST AT LEAST 12 HOURS NOISE FREE.
9. The MCU process:
9.1. Registration administration.
9.1.1. Participants show ID badge and identification card (passport) to the
registration officer(s).
9.1.2. The registration officer(s) will provide the required form to be filled.
Please return the filled form to the registration officer(s).
9.2. Vital Sign examination (weight, height, blood pressure, vision eyes,
pulse, respiration).
9.2.1. The officer will perform the vital sign examination.

9.2.2. The officer(s) will sign the form after the examination is completed.
9.3. Physical examination (by a doctor).
9.3.1. Participants go to the examination room.
9.3.2. Participants give true answers when history taking takes place.
9.3.3. Doctor will perform a physical examination. If participants are not willing,
they have to made informed consent of physical examination refusal.
9.3.4. Doctor will sign the form after the physical examination is completed.

9.4. Blood sampling (Laboratory).


9.4.1. Participants are required to fast for minimal 8 hours in advance (do not
eat and drink anything except drinking mineral water)
9.4.2. Participants are not allowed to consume drugs (except regular
medicines) and perform excessive physical exercise before blood test.
9.4.3. The officer(s) will draw the blood. Participants are expected to remain
calm (Please do not tense when the blood is drawn).
9.4.4. The officer(s) will sign the form after the blood sampling is completed.
9.5. Urine sampling (Laboratory).
9.5.1. Laboratory officer(s) will provide a labeled urine tube to be filled.
9.5.2. Participants go to the toilet, wash and dry hands thoroughly.
9.5.3. Participants remove the cap of the container and set it aside. Please do
not touch the inner surface of the cap as it will cause contamination with
bacteria that normally reside the skin.
9.5.4. Participants pass a small amount of urine into the toilet (not the cup).
Midway through urination, move the container into the stream of urine
and fill the container about half to two thirds full.
9.5.5. Participants replace the cap and tighten firmly.
9.5.6. Participants wash and dry hands thoroughly.
9.5.7. Participants deliver the container back to the laboratory officer(s).
9.5.8. The officer(s) will sign the form after the urine sampling is completed.
9.6. ECG (corresponding HIRADC mapping).
9.6.1. ECG is reserved for ages 30 and older.

9.6.2. Participants are required to inform the officer(s) if they have PEN on or
have done a by-pass heart surgery recently.
9.6.3. Participants are advised to shave chest hair.
9.6.4. Participants are required to take off jewelry (earrings / bracelets / rings /
necklace) and watch and to remove clothing from the waist up.
9.6.5. Participants lay flat on the ECG bed.
9.6.6. ECG officer(s) will rub alcohol and put sensors on the upper arms, lower
legs and chest.
9.6.7. The officer(s) will sign the form after the ECG test is completed.
9.7. X-ray examination.
9.7.1. Women who are pregnant (based on interviews) can not participate in x-
ray examination.
9.7.2. Participants are required to take off jewelry (earrings / bracelets / rings /
necklace) and watch and to remove clothing from the waist up.
9.7.3. The officer(s) will sign the form after the x-ray examination is completed.
9.8. Spirometry examination (corresponding HIRADC mapping).
9.8.1. Participants are advised to have enough rest.
9.8.2. Participants are advised to wear loose clothing.
9.8.3. Participants are required to stop smoke at least 8 hours prior
examination.
9.8.4. Participants are required to inform the officer(s) if they are taking
medication.
9.8.5. Participants are required to inform if they are suffering from respiratory
infections such as colds, coughs, colds and asthma.
9.8.6. Examination will not be performed if participants recently suffer from ear
infections or oral surgery.
9.8.7. The officer will inform the spirometry procedures.
9.8.8. The officer(s) will sign the form after the spirometry examination is
completed.
9.9. Audiometric examination (corresponding HIRADC mapping)
9.9.1. Participants are advised to clean their ear prior examination.

9.9.2. Participants are required to inform the officer(s) if they are taking
medication.
9.9.3. Participants are required to inform if they are suffering from ear, nose, or
throat (ENT) infection.
9.9.4. Participants are required to take off the earrings.
9.9.5. Participants are not allowed to use a headset or the like (MUST AT
LEAST 12 HOURS NOISE FREE).
9.9.6. Participants must pass a physical examination first.
9.9.7. Examination will not be performed if participants have ear wax or ENT
infection.
9.9.8. The officer will inform the audiometric procedures.
9.9.9. The officer(s) will sign the form after the audiometry examination is
completed.
9.10. Repatriation of data administration
9.10.1. Participants must return the result of the examinations are completed.
10. For those who wear spectacles, please bring spectacles.
11. Hand phone or other communication devices are recommended to be put in the
"Silent Mode" during the examination, and if it is needed to pick up the call please
do it outside clinic building.

MCU SERVICE WILL NOT BE SERVED if the participant does not follow the
above procedure.
