AFFIDAVIT OF UNDERTAKINGS

I, DARWIN V. GARCIA, of legal age, Filipino, married, a resident of Punta Biao, Cogon,
Digos City, Philippines, after having been duly sworn to in accordance with law, DO HEREBY
DEPOSE AND SAY THAT:

That I am the same identical person who caused the execution of this affidavit;

That I am the driver of the motorcycle with plate no. LE-9844 in the name of Jolito
Mandreza which was registered on April 13, 2009;

That I was attended by the law of LTO traffic enforcer.

That I DARWIN V. GARCIA, shall promise and undertake all the responsibility and liability
in the event that Jolito Mandreza; or her heirs or other persons, if there be any will institute cases
relative to this motor vehicle and that I will held myself responsibilities only, and free the LAND
TRANSPORTATION OFFICE of any liability;

I AM EXECUTING this Affidavit freely and voluntarily to attest the truth and veracity of the
foregoing allegations.
