AND TESTIFY FOR AND IN BEHALF OF THE CORPORATION.

“On motion duly seconded, it was RESOLVED, as it is hereby resolved, to

authorize ___________________________ to file a petition/ application before the
Land Transportation Franchising and Regulatory Board (LTFRB), Regional
Office No. _______________, for the issuance of a Provisional Authority and the
renewal of Certificate of Public Convenience (CPC) to operate TH Freight Truck
Service for FOUR (4) UNITS with plate nos. _________________________.

“RESOLVED, Further, that ____________________ is hereby authorized to

sign the said petition, pleading or any other document necessary for the
provisional authority and renewal of CPC, including the Verification and
Certification portion of the petition, and to appear and to testify before the
LTFRB during the designated trial dates therefor.”
