TEMPLATE USAGE GUIDELINES

CLASSROOM ASSESSMENT (CA) MATHEMATICS

INTRODUCTION
Classroom Assessment (CA) is a part of the component in the School-based Assessment (SBA). The implementation has started
since 2011 based on the Surat Siaran Lembaga Peperiksaan Bil. 3 Tahun 2011. CA was formerly known as School Assessment
(SA) which was implemented formally and summatively using various approaches and methods to identify pupils' overall
learning development.

A GENERAL INFORMATION
This CA Reporting Template consists of 5 sheets:
1. GUIDELINES
2. PUPIL'S PERFORMANCE RECORD
3. PUPIL'S REPORT (INDIVIDUAL)
4. PERFORMANCE LEVEL STATEMENTS DATA
5. GRAPHS

B TEMPLATE USAGE
Teachers have to complete the general information in the PUPIL'S PERFORMANCE RECORD.
The required details are:
1. Name and School Address
2.Teacher's Name and Class
3.Pupil's Name List, Identity Card Number and Gender
4. Administrator's Name
5.Administrator's Designation (Headmaster/ Principal)

C PERFORMANCE LEVEL
Pupil's Performance Level for every component in this template is recorded to report the pupil's learning development for

a certain period of the year (Mid-year / Final Year). Teachers only need to record the Performance Level in the PUPIL'S

PERFROMANCE RECORD and the pupil's individual report will be automatically generated in the PUPIL'S REPORT

(INDIVIDUAL) for printing. Analysis of the Performance Level (PL) is automatically generated in the GRAPHS sheet.

D PERFORMANCE LEVEL EVALUATION


1 The assessment is an ongoing process and the pupil's performance level is monitored continuously. This performance level

can be recorded in a record book, or other written forms; however for the parents' report, this can be recorded in the

template provided and reported twice a year which is during middle and end of the year.
2 This reporting template is made up of seven columns based on knowledge, skill and value constructs.
3 Teachers have to choose the option on the right on top of the Pupil's Performance Record sheet to make reports in this
template.
4 The report for knowledge, skill and value constructs will be executed in the middle and end of the year.
5 The Performance Level is executed based on every rubric referring to the knowledge, skill and value constructs found in
the Performance Level Statements Data.
6 Teachers have to complete the teaching and learning of each theme before evaluating the Performance Level (PL) for each
theme.
7 Teachers have to refer to the Performance Standard (PS) found in the Standard Curriculum and Assessment Document
(SCAD) to evaluate the PL for each theme as the definition of PS can help teachers to determine the pupil's performance.
