To whom it may concern:

The undersigned is a 3rd yr Law student of De La Salle Lipa. I am conducting a research

on the “Legal Implications on the Commission on Human Rights’ Inaction among the Family
Members who developed Post-Traumatic Stress Disorder on Account of Extrajudicial Killing”
for the partial fulfillment of the Doctor of Jurisprudence degree. In line with this, I would like to
extend an invitation to you to participate in this research.

If you decide to participate, an interview would be arranged at a time and place of your
convenience. The interview would last about an hour. During this interview, I will be asking
questions regarding the abovementioned research topic. The information will be kept private and
confidential. You will be given a false name and identifiable information will never be presented.
I will not pass on your details to any organization or company. This is a personal research for
academic purposes and is not connected with the government.

This interview will take approximately 60 minutes, but this depends on how much you
have to say. There are no right or wrong answers. What is important are your opinions. After the
interview, I may contact you at a later date to clarify certain points or invite you to volunteer to
participate in taking photographs of your daily life. Participation in this research is completely
voluntary and you may choose to withdraw from the research at any time or not answer questions
that you do not feel comfortable answering.

If you have any further questions about the research, please feel free to contact me via
email at niacolinemacalamendoza@yahoo.com or on my mobile at 0947-620-5820.
