1. A 38 year old man is transported to the emergency department after having fainted at home.
The patient is a recent immigrant to the United States from equatorial Africa. During the past
year he has become progressively weak and lethargic. He had tuberculosis 2 years ago, which
was managed medially. The patient has had vomiting and diarrhoea for the past 24 hours, and
his family reports a weight loss of about 15 pounds during the past 3 months.
2. A 35 year old woman comes to her physician’s office with the complaint of recent rapid weight
gain and excessive sweating. What initiated her visit was a recent pain attack that frightened
her. Her face looks swollen compared with the rest of her body. She complains of recent
weakness, backaches, and headaches, and her periods have lately been irregular. Over the
past month, she has noticed frequent bruising with slow healing. She is not on any birth
control or using any medication except for acetaminophen for the headaches.
3. A 45 year old woman comes in for her regular check-up and complains about a constant thirst
and having to get up frequently in the night to urinate. The patient has a history of high blood
pressure. It is possible that she may be noncompliant with her medications because her blood
pressure has remained regardless of the medications she was prescribed or the dosage taken.
Diagnosis: Pheochromocytoma
5. A 33 year old woman comes to her primary care physician complaining of amenorrhea,
headaches, profuse sweating, and joint pains associated with changes in the sizes of her hands
and feet. The patient reports massive growth of her hands and feet as well as protrusion of
her lower jaw. She believes the shape of her face has changed, becoming sharper. She also
complains pf sudden weakness, even when just ‘carrying the clothes hamper upstairs.’ She
first noticed these changes about 6 months ago when she was home for the holidays and saw
some high school and early college pictures of herself. Her mother brought to her attention
how features seemed much larger. After that she says she became more aware of her bodily
changes. She came in because she says she knows something is wrong and that there is
definite distinction from how she looks now from how she used to look. Her husband and she
are also trying to conceive and have been unsuccessful due to amenorrhea for the past 8
months.
Diagnosis: Acromegaly
6. A 42 year old man comes to his physician because of a decrease in sexual desire and erectile
dysfunction. The patient noticed visual problems and headaches during the past month. The
decrease in sexual desire began about 2 months ago and was accompanied by erectile
dysfunction.
Diagnosis: Hyperprolactinemia
7. A 28 year old woman complains to her obstetrician of amenorrhea and milk discharge from
her nipples. The patient was involved in a motorcycle accident 3 months ago. Injuries were
mostly superficial, but the impact was sufficient to break her helmet. The patient indicated
she has not “felt right since the accident”. She is constantly tired, has gained 5 pounds, and
drinks and urinates more than before the accident.
Diagnosis: Panhypopituitarism
8. A 30 year old woman visits her primary care doctor with concerns about a growth on her neck.
The patient also complains about anxiety and irritability. She has noticed a 6-pound loss of
weight over the past 2 months despite an increase in her appetite. She also feels
uncomfortable when in warm room.
9. A 48 year old woman visits her family practitioner complaining of fatigue, weakness, and
weight gain. The patient also states that she experiences frequent muscle cramping and
always feels cold. When questioned, she indicates that she has experienced an increase in
constipation and headaches during the past 2 months.
10. A 58 year old woman is referred to an internist due to high calcium levels found during
treatment for a fracture of her femur. The patient’s femur was fractured at the midshaft by a
compression. The injury should not have been severe enough to break the bone. During
treatment, the patient was noted to have high calcium levels and was referred for further
study.
