SUBJECT: Evaluation Summary: Intramurals 2017
DATE: April 27, 2018

The Office of Student Affairs is responsible for meeting the needs of students’ recreational
pursuits and implementing a program of activities which contribute to the physical, mental and
social health of our students.
The evaluation form (Attached as Exhibit A), appraised the intramural sports programs at
Osias Colleges, Inc. The survey type form was used for the evaluation. The total number of
respondents consisted of 250, 50 students Business Admin., 50 students Criminology, 50 students
Education ,50 students Senior high Department and 50 students from the Junior high Department.
With the assistance of Ms. Cyrah Melocotones, the Guidance Staff, the student activity assessment
and evaluation form was used for the data collection. The statistics used for data analysis is the
Weighted Average to get the result. The result showed that the intramurals was POOR with regard
to its timely and appropriate information dissemination to promote the activity and to encourage
maximum participation. The activity generally received a FAIR feedback, based from the
Evaluation Summary (Attached as Exhibit B). It was therefore suggested that the school should
provide better facilities and equipment’s, sound system/ speakers to attract more interests in
intramural sports. The comments/suggestions are indicated in the summary for reference (Attached
as Exhibit C).
In view of the findings of this evaluation, the following recommendations are presented:
1. Provisions of facilities and equipment are essential and are of vital importance in the
conduct of sports program.
2. The open space for the holding of ballgames should be covered (COVERED COURT) to
encourage mass participation.
3. Scheduling is a significant problem in sports, thus, teachers should show more interest in
intramural sports and help organize the program appropriately.
4. There is a need to employ more qualified teachers for physical education.

SUBJECT: Evaluation Summary :Foundation Day 2018

The Office of Student Affairs is responsible for meeting the needs of students’ recreational
pursuits and implementing a program of activities which contribute to the physical, mental and
social health of our students.
The school celebrates its annual Foundation Day every month of March. The Foundation
Day celebration highlights the talents of the students in different fields such as singing, dancing,
hosting, field demonstration and pageantry. Aside from the showcase of talents, the Foundation
Day 2018 was also a fund raising event with a purpose. The evaluation form (Attached as Exhibit
A), appraised the Foundation Day programs at Osias Colleges, Inc. The survey type form was used
for the evaluation. The total number of respondents consisted of 250, 50 students Business Admin.,
50 students Criminology, 50 students Education, 50 students Senior high Department and 50
students from the Junior high Department. With the assistance of Ms. Cyrah Melocotones, the
Guidance Staff, the student activity assessment and evaluation form was used for the data

showed that the Foundation Day got a VERY GOOD remark with regard to the working
committees having clearly defined functions and responsibilities and clearly identified lines of

OPEN GATES, putting up of booths from the different departments to encourage attendance and
improve the physical arrangement to best suit the program. The comments/suggestions are
indicated in the summary for reference (Attached as Exhibit C).

The Office of Student Affairs is responsible for meeting the needs of students and parents
in school programs. One of which is the GRADUATION DAY, a culmination of years of student
life at Osias Colleges. Graduation day is one of the best days of their lives. hE is responsible for
meeting the needs of students’ recreational pursuits and implementing a program of activities
which contribute to the physical, mental and social health of our students.
The evaluation form (Attached as Exhibit A), appraised the Graduation programs at Osias
Colleges, Inc. The survey type form was used for the evaluation. The total number of respondents
consisted of 40 Employees from the different offices. With the assistance of Ms. Cyrah
Melocotones, the Guidance Staff, the student activity assessment and evaluation form was used for
the data collection. The statistics used for data analysis is the Weighted Average to get the result.
