for the month. Columns for "Cumulative as of Previous Month" require the figures in "cumulative total reported from previous month". Prepared and Submitted by:

3. Teachers who are handling advisory class shall be reported. REY S. VISTAL
4. Small school that has one section per grade/year level are not required to fill the columns "Name of Adviser, Grade/Year Level & Section". (Signature of Adviser over Printed Name)
Instead, they will only accomplish the summary column per grade/year level.
