PETITIONER, ___________________________, unto this Honorable Court, most

respectfully states that:

1. He is of legal age and a Filipino Citizen;

2. He is presently residing at San Rafael, Roxas, Isabela and residing therein since
______ and maintains a regular place of work in 145 Mother Ignacia Avenue,
South Triangle, Quezon City, Metro Manila;
3. He has satisfactorily completed the required course in law at San Beda College in
__________;
4. He has passed the __________ Bar Examinations and admitted to practice law in
the Philippines on _______. Copy of his Certificate of Bar Membership is hereto
attached as Annex “A”;
5. He is a member of the Philippine Bar in good standing with Clearances from the
Office of the Bar Confidant of the Supreme Court and the Integrated Bar of the
Philippines. Copy of said clearances are hereto attached as Annex “B” and Annex
“C”, respectively;
6. The undersigned has not been charged nor convicted in the first instance of any
crime involving Moral Turpitude; and
7. He desires to be commissioned as Notary Public for and in City of Quezon,
Metro Manila and possesses all the qualifications and none of the
disqualifications for said officer.

PRAYER

1. The Petition for Commission as Notary Public filed by the undersigned be acted
favourably; and
2. Petitioner be commissioned as a Notary Public in the City of Quezon, Metro Manila.
