INFORMATION

The undersigned City Prosecutor, upon sworn complaint originally filed by the
offended party, accuses GEORGE T. CONWAY of SERIOUS PHYSICAL INJURIES,
committed as follows:

That on or about four o’clock in the afternoon of February 9, 2019 in the city of
Davao, province of Davao del Sur, Philippines, within the jurisdiction of this court, the
said accused, did then and there willfully, unlawfully, and feloniously attack, assault, and
used personal violence upon one GABRIEL S. IGLESIAS by then and there punching him
without warning and striking him with a jack wrench, thereby inflicting upon the latter
concussion and bruises on the chest and shoulders, which caused the illness of the said
GABRIEL IGLESISAS for more than 30 days, and incapacitated him from performing his
customary labor for the same period of time

CONTRARY TO LAW.

that there is a reasonable ground to believe that a crime has been committed and that
the accused is probably guilty thereof; that the accused was informed of the complaint
and the evidence submitted against him and was given an opportunity to submit
controverting evidence.

That a preliminary investigation has previously been conducted in this case under my
direction, having examined the witnesses in accordance with the provisions of R.A. No.
5180, as amended by P.D. No. 77, Dec.6, 1972 and P.D. 911, March 23, 1976 and as
implemented by Dept. of Justice Circular No. 74, series of 1967 and Circular No. 23,
series of 1975
