Requirements in Death Claim (Livestock)

Please provide the following:

Notice of Loss
LIVESTOCK DEATH CERTIFICATE (Duly signed & sealed by the Local Civil Registrar)
Police Report (if the cause of death is accident)
Veterinary Disease Report -VDR (Duly signed by the Municipal Veterinarian)
Loss Report (Duly signed and accomplished by the assured)
Photographs of dead animal (front and back view) original
Other Documents such as sworn affidavit of two (2) disinterested persons
as maybe required by PCIC.
Animal Dispersal Certificate (Certified Machine Copy)
