TRAINING CONTRACT

This contract is made and entered into this _______ day of __________________, 20___ by and
between:

L3D ARTS AND CRAFTS, a juridical person, duly existing and constituted under the laws of the
Philippines with office address at Poblacion, Valencia City, Bukidnon, represented by its Chief
Executive Office LEVY G. DAVID hereinafter referred to as the FIRST PARTY,

-and-

__________________________, of legal age, Filipino, and a resident of

________________________________________, a trainee of L3D ARTS and CRAFTS
hereinafter referred to as the SECOND PARTY.

WITNESSETH:That

WHEREAS, the SECOND PARTY agrees to be a trainee by the FIRST PARTY under the following
conditions:

I. TRAINING DURATION

1. That the SECOND PARTY shall undergo three (3) months training which will consist of
one (1) month seminar and two (2) months field exposure training related to his/her
assigned work by the FIRST PARTY;

II. TRAINING MODULE

2.1 That the training includes modules of arts, painting and teaching;

III. TRAINEE STIPEND

3.1 That the SECOND PARTY during the duration of the training will be entitled of a
stipend in the amount of One Thousand Five Hundred (Php 1,500.00) Pesos per month,
Philhealth and Social Security System remittance, and free lunch during the training;

IV. SEMINAR CERTIFICATES

4.1 That the SECOND PARTY will be awarded with seminar certificates upon completion
of the training program;

V. TRAINING PRE-TERMINATION

5.1 Should either of the parties wish to be pre-terminate the contract for any reason,
EXCEPT violations of its provisions, the party wishing the same shall notify the other at
Lest one month before the ended termination;

5.2 Either party may pre-terminate this contract at any time, after due process and
proper notice is served;

5.3 In case of pre-termination of contract in breach of the preceding sections, the party
that breached will be held liable to pay damages to other party to the amount as the
court may deem appropriate under the circumstances;

VI. RETURN OF SERVICE

6.1 The trainee agrees that he/she is obliged to render return service after training for a
period of at least Two (2) years;

VII. OTHER PROVISIONS

7.1 Considering that the L3D ARTS AND CRAFTS is an Arts institution, the
SECOND PARTY, just like other trainees, is mandated to obey the Office Rules, which
essentially form part and parcel of this contract.

VIII. AMENDMENTS

8.1This contract hall be subject to additional terms and conditions and amendments
that may be deemed necessary in the future and agreed upon by both parties.
