AFFIDAVIT OF SELF-ADJUDICATION BY SOLE HEIR

I, JOHN MICHAEL SAGUN TIPON, Filipino, of legal age, single, with residence address
at Block 204 Lot 8 Purity St. Phase -7 Las Villas Sto.Nino, Bahay Pare, Meycauayan City,
Bulacan under oath, depose and say:

1. That I am the sole heir of the late CARMELITA SAGUN TIPON married to deceased
EDGAR CORRALES TIPON there being no other ascendants, descendants and collateral
relatives, surviving, who died intestate in Meycauayan City, Bulacan last January 31,
2019. A copy of the Certificate of Death is hereby attached as Annex A;

2. That I was born on September 30, 1985 at the Our Lady of Lourdes Hospital P. Sanchez
St. Sta. Mesa Manila at around 12:25 am. On the basis of my Certificate of Live Birth
my parents are Carmelita Sagun and Edgar Tipon and both of them celebrated their
marriage on December 8, 1984 at Santissimo Parish Church situated at the University
of Sto. Tomas. A copy of the Certificate of Live Birth is hereby attached as Annex B;

3. That at the time of her death, she left a real property which was mortgaged at the
National Home Mortgage Finance Corporation herein referred to as (NHMFC) covered
by Transfer Certificate of Title No. T-307997, issued by the Register of Deeds for City
of Trece Martires, Province of Cavite, which is now released, fully paid and its title free
from all liens and encumbrances, will be claimed at the head office of NHMFC. This is
particularly described as follows:

A parcel of land ( Lot 13, Blk. 20 of the cons.-subd, plan Pos-004133, being a portion
of the cons. of Lots 3345-A (LRC) Ped-82057, 2995-A, 2995-B (LRC) Psd-227323,
2996-A, 2996-B, 2996-114769, 3343, 3344, Flo-2316, Sta. Cruz de Malabon Estate
and Blk. 20, Pcs-04-003816, L.R.C. Rec. No. ). situated in the Bo. of Lapidario, City of
Trece Martires, Is. of Luzon. Founded on the NW., along line 1-2 by lot 15, Blk. 10; on
the NE., line 2-3 by Road lot 9; on the SR. along line 3-4 by lot 11; is registered in
accordance with the provisions of the Property Registration Decree in the name of
EMERITO C. TIPON, of legal age, Filipino, married to Dulce C. Tipon, and CARMELITA S.
TIPON, of legal age, Filipino, married to Edgar C. Tipon, equal share each. as owner
thereof in fee simple.

4. That as the sole heir of the late Carmelita Tipon I acknowledge and recognize the
ownership of Emerito C. Tipon of the other half of the property being described as
duplex;
5. That Emerito C. Tipon is the first cousin of my father Edgar C. Tipon. Emerito’s parent
Regino Tipon is the sibling of Elpidio U. Tipon, father of my parent Edgar C. Tipon;

6. That as a result of their blood relationship, I am the nephew of Emerito C. Tipon;

7. That the deceased left no debts or obligations regarding this property;

8. That as a sole heir, I am authorized to claim and receive the Transfer Certificate of
Title, free from all liens and encumbrances at the head office of NHMFC at Filomena
Building, Amorsolo St. Makati City in its office hours from 8 am to 5 pm during
weekdays;’

9. That pursuant to Rule 74, Sec. 1 of the Rules of Court, I hereby adjudicate unto myself
the above mentioned property including all improvements built thereon and accounts
by means of this affidavit and files the same with the Register of Deeds for the City of
Trece Martirez, Province of Cavite, and other concerned agencies with the request that
said adjudication be made effective without judicial proceedings as prescribed by the
aforementioned Rules of Court;

10. I am executing this instrument, to attest to the foregoing facts and circumstances and
for all legal intents and purposes it may serve.
