RESOLUTION No. 10
(Series of 2018)

“A RESOLUTION AUTHORIZING THE BARANGAY TREASURER MS. JUANITA O. MENDOZA TO

WITHDRAW THE AMOUNT OF THREE THOUSAND PESOS (P3000.00) AS FINANCIAL MEDICAL
ASSISTANCE FOR BARANGAY RECORD KEEPER (BRK) OF BARANGAY MABAYUAN, MR. ALVIN G.
CUEVAS”.

WHEREAS, Mr. Alvin G. Cuevas, Barangay Record Keeper of Barangay Mabayuan was check
up and treated in Baypoint Hospital at Subic Bay Freeport Zone due to his kidney problem (kidney
stone);

WHEREAS, Barangay Officials, employees, staffs and other appointive personnel are the life-
blood and heart of the Barangay Government for its successful business and undertaking;

 WHEREAS, the Barangay Administration in accordance with the law, provides all necessary
assistance to ensure the well-being and the best interest of its personnel especially in time of
necessity, emergency, misfortune as well as tragedy;

NOW THEREFORE, upon the motion of Kagawad Mike R. Sagun duly seconded by Kagawad
Nenita S. Jao with the unanimous accord of all the Sangguniang Barangay of Barangay Mabayuan;

BE IT RESOLVED, a Resolution authorizing the Barangay Treasurer to withdraw the amount of

Three Thousand Pesos (P3000.00) as financial medical assistance for Barangay Record Keeper (BRK),
Mr. Alvin G. Cuevas;

RESOLVED FURTHER, that copies of this resolution be forwarded to City Accounting

Department, Department of Interior and Local Government (DILG), the Sangguniang Panglungsod
and other offices concerned for their information and records;

APPROVED UNANIMOUSLY this 3rd day of August, 2018 .
