India’s first exhaustive study on annual snake-bite deaths, the Million Death Study (April
2011), has revealed the annual death figures to be as high as 46,000 while the total number
of annual snake-bites are estimated to be around 14,00,000. India has long been thought to
have more snake-bites than any other country. The first ever direct estimates were drawn
from a national mortality survey of 11 lakh homes. The estimated total of 46,000 national
snake-bite deaths constitutes about 5% of all injury deaths and nearly 0.5% of all deaths in
India. It is more than 30-fold higher than the number declared from earlier official hospital
returns. The underreporting of snake bite deaths has a number of possible causes, most
importantly, it is well known that many patients are treated and die outside health facilities
especially in rural areas.
As per the study, states with a high-prevalence of snakebite deaths are defined as those
with more than 1 crore people where the annual snakebite death rate exceeds 3 per
1,00,000 population. Gujarat has an annual snake-bite death rate of 3.5 per 1,00,000
population and is one of the 13 states with high prevalence of snake-bite deaths. This means
around 2,100 people die in Gujarat every year from 1,34,400 bites (non-fatal snake-bite to
death ratio is 64:1 as per hospital records during the study); even if the ratio is halved it
comes to 67,200 bites every year.
This publication aims to pass on a digest of available knowledge about all clinical aspects of
snake-bite to medically trained personnel. The guidelines are intended for medical doctors,
nurses, dispensers and community health workers who have the responsibility of treating
victims of snake-bite. They aim to provide sufficient practical information to allow medically
trained personnel to assess and treat patients with snake-bites at different levels of the
health service.
Introduction:
There are more than 250 species of snakes in India, 52 of which are venomous (Poisonous).
There are 4 Families of Venomous snakes:
These four species are referred to as the Big Four snakes of India as they are
collectively responsible for most snake-bite deaths in India. The estimated total of
46,000 national annual snake-bite deaths constitutes about 5% of all injury deaths
and nearly 0.5% of all deaths in India.
Snake-bite preventive strategies by rural communities and effective post bite clinical
management are key mitigation measures.
How to treat:
Do’s:
1. Reassure the Patient that all the snakes are not venomous and even if venomous,
treatment is available.
2. Immobilize the affected part with splint or bandage.
3. Make the victim lie flat with affected limb below heart level.
4. Remove shoes, tight clothes, rings, jewellery or watches.
5. Be prepared for cardiorespiratory resuscitation. (CPR)
6. Maintain airway, breathing and circulation. (ABC)
7. Administer Inj. Tetanus toxoid IM
8. Give Antibiotics if skin is breached or localized necrosis.
9. Try to identify the snake if available or ask the relatives who have seen that.
10. Monitor the patient for 24 hours for late symptoms.

Do’s:
1. Do not suddenly remove tourniquet which can cause sudden flushing of venom in
circulation.
2. Watch for symptoms whether hemotoxic or neurotoxic.
3. Run Hb, total count, platelets, PT/aPTT, Urine R/M, Urea, Creatinine, Sodium,
Potassium, ECG and Chest X-ray, ABGA.
4. Run 20 minute whole blood clotting test to detect coagulopathy.
5. Administer Anti snake venom.
DOSE:-
8 to 10 vials of ASV is given over 1 hour as it removes most of the unbound toxin.
No next dose to be given till the next Clotting test done after 6 hours.
If WBCT still > 20 minutes repeat dose of 5-10 vials given.
If 20 vials given then no further ASV should be given assuming that all the unbound
toxin has been removed.
Be ready for any anaphylactic reaction.
Inj. Adrenaline + Hydrocortisone (100 mg) + Antihistaminic (Phenylephrine) should
be available before administering ASV.
Administer Inj. Neostigmine 0.5-2 mg i.v if there are neurological symptoms and
repeat every half hourly for next 8 hours if improvement occurs.

Principles of first-aid
First-aid treatment is carried out immediately or very soon after the bite, before the
patient reaches a dispensary or hospital. It can be performed by the snake-bite victim
himself/herself or by anyone else who is present and able.
Aims of first-aid
Attempt to delay systemic absorption of venom.
Preserve life and prevent complications before the patient can receive medical
care
Control distressing or dangerous early symptoms of envenoming.
Arrange the transport of the patient to a place where they can receive medical
care.
ABOVE ALL, AIM TO DO NO HARM!
Release of tight bands, bandages and ligatures: Ideally, these should not be released
until the patient is under medical care in hospital, resuscitation facilities are available
and ASVS treatment has been started.
Traditional tight (arterial) tourniquets are not recommended. To be effective, these had
to be applied around the upper part of the limb so tightly that the peripheral pulse gets
occluded. This method can be extremely painful and very dangerous if the tourniquet
was left on for too long (more than about 40 minutes), as the limb might be damaged by
ischemia. Tourniquets have caused many gangrenous limbs.
Snake-bite is one of the more serious health issues, especially in India where deaths
due to snake-bites are estimated well over 46,000 every year. Even though actual
envenomation takes place less often, it is best to stay prepared for an emergency.
Make sure the victim and others are at a safe distance away from the snake; do
not attempt to capture the snake, get the victim and others away from the snake
Try to memorize the snake’s appearance (from a safe distance!)
Remove watches / rings / other jewellery from the bitten part
Keep the victim calm and reassured, do not panic, as trivial as it may sound, it
makes everything from the victim's ability to deal with the situation to your
ability to make decisions better
Try to immobilize the bitten limb; do not make the victim run and avoid making
him/her walk if possible
Do not waste any time and arrange for a quick transport to the nearest hospital
treating snake-bite cases, as safely and comfortably as possible
If possible, note the time of bite and progression of symptoms
Describe the snake and the whole incident to the attending doctor
The above 4 species account for most snake-bite deaths in India. Please be extra careful if
you are around these.
In case of a snake-bite, call 108 from your phone. The emergency response service will
immediately dispatch an ambulance with trained paramedics.
In the house:
Do not keep livestock, especially chickens, in the house, as snakes may come to
hunt them. Store food in rat-proof containers. Regularly check houses for snakes
and, if possible, avoid those types of house construction that will provide snakes
with hiding.
If at all possible, try to avoid sleeping on the ground. If you have to sleep on the
ground use mosquito net that is well tucked in under the mattress or sleeping
mat.
In the yard:
Try not to provide hiding places for snakes. Clear termite mounds, heaps of
rubbish, building materials etc. from near the house.
Do not have tree branches touching the house. Keep grass short or clear the
ground around your house and clear low bushes in the vicinity so that snakes
cannot hide close to the house.
Keep your granary away from the house, it may attract rodents that snakes will
hunt.
Take great care when clearing vegetation, raking dry leaves in outdoors.
Use a light (torch or lamp) when you walk outside the house or visit the latrine at
night.

પ્રાથમિક ઉપચાર સાપ ડંખે એટલે તરત જ અથવા થોડી વારિાં કરવાિાં આવે છે ,
દરદી કોઇ ડડસ્પેન્સરી અથવા હોસ્સ્પટલિાં પહોંચે તેની પહેલા. આ ડંખ પાિેલ વ્યસ્તત
પોતે કરી શકે અથવા એવી કોઇ પણ વ્યસ્તત કરી શકે, જે હાજર હોય, અને તે કરવા
સક્ષિ હોય.
કિભાગ્યે, િોટા ભાગની પરં પરાગત, લોકમપ્રય, ઉપલબ્ધ અને પોસાય તેવી પ્રાથમિક
છે : ડંખના સ્થળ પર અથવા જયાં ડંખ પડયો હોય તે અંગિાં ચીરો/ કાણાં પાડવા,
ઘાિાંથી ઝેર ચ ૂસવાનો પ્રયત્ન કરવો, સપપ પત્થરનો ઉપયોગ કરવો, હાથ-પગ પર
કસીને પાટા િાંધવા (ટોનીકેટ), વીજળીના ઝાટકા આપવા, ડંખના સ્થાન પર રસાયણ,
જડીબુટ્ટિઓ અથવા િરફ લગાવવા અથવા નાંખવા. સ્થામનક લોકોને પરં પરાગત
િેળવવાિાં િોડું કરવાિાં કે કોઇ નુકસાન પહોંચાડવા દે વાિાં આવવુ ં જોઇએ નહીં.
સખત બેન્ડ, પટ્ટીઓ અથવા પાટા ખોલવા: આિ તો, તેિને જયાં સુધી દરદી
હોસ્સ્પટલિાં સારવાર િેળવતો થાય, શ્વાસ ફરી ચાલુ કરવાની સગવડ હોય અને
એએસવીએસ ઉપચાર ચાલુ કરવાિાં આવ્યો હોય, ત્યારે જ છોડવા જોઇએ.
1. શક્ય એટલી ઝડપથી એક 2. પટ્ટી તિે િચકોડ પર બાાંધો 3. શક્યે એટલે ઊંચે સુધી બાાંધો.
પહોળી દબાણ પટ્ટી સાપ કરડયો એટલી સખત હોવી જોઇએ.

પર્િાાં પહેરેલ કપડાાં કાઢશો નહીં, કરવુાં જોઇએ નહીં.

શકે. ડાંખ પડેલ પર્ સ્થથર રાખો.

ટોનીકેટ ન િાંધો

ઉપરોતત કાયો અસરકારક નથી હોતા અથવા તે કરવા અત્યાંત જોખિી હોય છે . ખરે ખર
ખાતરી કરો કે ડંખ પાિેલ વ્યસ્તત અને અન્યો સાપથી સુરબક્ષત અંતરે હોય;
સાપને પકડવાનો પ્રયત્ન ન કરો, ડંખ પાિેલ વ્યસ્તત અને અન્યોને સાપથી દૂ ર
લઇ જાવ
ડંખ િારે લ ભાગિાંથી ઘડડયાળો / વીંટીઓ / અન્ય દાગીના કાઢી નાંખો. ડંખ
ુ
પછી ઝડપથી સોજા આવી જાય છે અને આવી વસ્તઓ લોહીના પ્રવાહને રોકી
સાવ નકાિી વાત લાગે, આનાથી ડંખ પાિેલ વ્યસ્તતની પડરસ્સ્થમતનો સાિનો
કરવાની ક્ષિતાથી લઇને તિારી મનણપય શસ્તત, િધા પર સકારાત્િક અસર પડે
છે
જે અંગ પર સાપ ડંખ્યો હોય, તેન ુ ં હલનચલન િંધ કરવાનો પ્રયત્ન કરો; ડંખ
પાિેલ વ્યસ્તતને દોડાવો નહીં અને શક્ય હોય તો તેઓને ચલાવવાનુ ં પણ ટાળો
સિય ન િગાડો અને શક્ય એટલા સુરબક્ષત અને સગવડપ ૂણપ સાધન વડે
કરો.
સારવાર કરનાર ડોતટર આગળ સાપ અને આખી ઘટનાનુ ં વણપન કરો
આ ફતત સંપકપ િાટે િહત્ત્વની વ્યસ્તતઓ, નંિરો, જેના પર પહેલા જાણ કરાવી જોઇએ,
સાપના ડંખનો ઇલાજ કરતી સૌથી નજીકની હોસ્સ્પટલનુ ં નાિ/સરનામુ,ં શ્રેષ્ઠ પડરવહન
અને સંિમં ધત મવગતો, ડોતટરનુ ં નાિ/ નંિર વગેરે હોઇ શકે. સુમનમિત કરો કે સંકટની
સ્સ્થમતિાં કુટુંિના િધા જ સભ્યો અને મિત્રો તેિની ભ ૂમિકાને િરાિર સિજે છે .

સાપ ડંખે તો તિારા ફોન પરથી 108 ને ફોન કરો. સંકટ સિયે પ્રમતભાવ સેવા તરત
જ તાલીિ પાિેલ પેરાિેડડતસ સાથેની એમ્બબ્યુલન્સ િોકલી આપશે.
મશક્ષણ: તિારા સ્થામનક સાપોને જાણો, જાણો કે તેઓ કેવા સ્થળોએ રહેવાનુ ં અને
છુપાવાનું પસંદ કરે છે , વષપના કયા સિયે અને ડદવસ/રાતના કયા સિયે
અથવા કેવા િોસિિાં તેઓ સૌથી વધુ સડિય હોય છે .
વરસાદ પછી, પ ૂર દરમિયાન, કાપણીના સિયે અને રાતના સિયે ખાસ ધ્યાન
રાખો.
સરખા પગરખા અથવા બ ૂટ અને લાંિા પેન્ટ પહેરો, ખાસ કરીને અંધારા અથવા
ઝાંખરાિાં ચાલતા હો ત્યારે . પગરખા પહેરતા પહેલા હંિેશા તપાસો!
રાતના સિયે ચાલતી વખતે ટોચપ/દીવાનો ઉપયોગ કરો.
શક્ય હોય ત્યા સુધી સાપોથી દૂ ર રહો, િદારીઓના ખેલ કરતા સાપોથી પણ.
ક્યારે ય સાપને પકડો નહીં, અથવા ડરાવો નહીં કે તેિના પર હુિલો ન કરો
અને ક્યારે ય જાણી જોઇને કોઇ સાપને ખ ૂણાિાં કે િંધ સ્થળે ફસાવો નહીં.
ઘરિાાં:
ઘરિાં જીવતા પ્રાણીઓ, ખાસ કરીને િરઘાં ન રાખો, કારણ કે સાપ તેિનો
મશકાર કરવા આવી શકે છે . ખાવાનુ ં ઉંદર ન પહોંચી શકે તેવા વાસણોિાં ભરો.
મનયમિતપણે ઘરિાં સાપ તો નથીને, તેની ખાતરી કરો, અને શક્ય હોય તો એવુ ં
ઘર િનાવવા કે તેિાં રહેવાનુ ં ટાળો, જેિાં સાપને છુપાવાની જગ્યા િળી શકે.
જો શક્ય હોય, તો જિીન પર સ ૂવાનુ ં ટાળો. જો તિારે જિીન પર સ ૂવુ ં પડે તેિ
હોય, તો િચ્છરદાનીનો ઉપયોગ કરો અને તેને ગાદલા અથવા શેતરં જી નીચે
સરખી રીતે દિાવો.
વાડાિાાં:
સાપને સંતાવાનુ ં સ્થાન ન આપવાનો પ્રયત્ન કરો. ઉધઇના દર, કચરાના
ઢગલા, કાટિાળ, વગેરે ઘરની આસપાસથી સાફ કરો.
ઘરને અડતી હોય તેવી ઝાડની ડાળો ન રાખો. ઘાસ ટૂંકુ રાખો અથવા તિારા
ઘરની આસપાસની જિીન સાફ રાખો અને આજુિાજુના નીચા ઝાંખરા કાઢી
નાંખો, જેથી સાપ ઘરની નજીક ન સંતાઇ શકે.
તિારો અનાજનો ભંડાર ઘરથી દૂ ર રાખો, તેનાથી એવા નાના પ્રાણીઓ
આકમષિત થઇ શકે છે , જેનો સાપ મશકાર કરે છે .
વનસ્પમત કાઢતી વખતે, અથવા સ ૂકાયેલાં પાંદડા ઉપાડતી વખતે ખ ૂિ કાળજી
રાખો.
રાતના સિયે ઘરની િહાર ચાલતા હો અથવા શૌચાલયિાં જતા હો તો પ્રકાશ
(ટોચપ અથવા દીવો) રાખો.
