1 lot Hauling, transport, treatment and disposal of the expired chemicals, laboratory waste, expired and
used veterinary biological products (vaccine)
Inclusion : Scope of Service
1. Inorganic Waste: 235 bottles (2.5L/ bottle for A & B)
A. Calcium waste- 2.5L bottle content are as follow:
1:3 HCI (one ml Hydrochloric acid mixed with 3ml water), and 2 drops of Nitric Acid
B. Phosphorous waste- 2.5L/ bottle contents are follows:
1:3 HCI ( one mL Hydrochloric acid mixed with 3mL water) Ammonium Molybdovanadate reagent 9contains
40 g Ammonium Molybdate +4 water; 250 mL Perchloric Acid (70%); 2 g Ammonium Metavanadate mixed
in 2L volumetric flask diluted with distilled water).
C. Expired Trace Mineral and Heavy Metal Standard, its concentration is 1000ppm- its content are; Cupper,
Lead, Manganese, Magnesuim, Potassuim, Soduim and Zinc, Arsenic, Cadmium, Lead and Mercury. ( 20
plastic bottles)
D. Lactose analysis wastes - its content are as follows: Saturated Picric Acid (16g Picric acid + 904 mL
distilled water) and 25% Na2CO3 (1mL/sample ).( 10 bottles)
2. Oragnic Waste : (20 bottles for A & B)
A. HPLC- (High Pressure Liquid Chromatography), waste contents are as follows:
Acentontrile -CAN; Methanol (Methyl Alcohol 0; Tri-Ethly Amine (TEA); Acetic Acid (HC2H3O2); Hexane salt;
Orthophosphoric Acid 9H3PO4; Ammonium Hydroxide (NH4OH); and potassium Dihydrogen Phosphate
(KH2PO4) for VDAL.
B. Organic Wastes for Toxin Laboratory-contents are: Acetonitrile, Methanol and water
C. Expired Organic Reagents, some are too old and unlabeled while others are labeled (70 bottles)
3. Expired Veterinary Biological Products (VACCINES) 35 sacks
Note Service Provider have an accreditation on handling / hauling of laboratory waste etc…

ADDITIONAL REMINDER/ REQUIREMENT:

A. In compliance with the DBM Circular Letter No. 2013-16 dated December 23, 2013, regarding the
“Implementation of the Expanded Modified Direct Payment Scheme (ExMDPS) For Accounts Payable Due
Creditors/Payees of All National Government Agencies”, we are requesting all participating bidders/suppliers
to submit together with their quotation the following information:

1) Account Name
2) Account Number
3) Name of Bank and its Branch (*Preferably Landbank of the Philippines)
4) Tax identification Number (TIN)

*It is also reminded that bank transfers, if creditor’s account is maintained outside the agency’s MDS-GSB,
where corresponding bank charges shall be borne/paid by the creditor/payee concerned.

B. Pursuit to the 2016 Revised Implementing Rules and Regulations of the Republic Act 9184, under ANNEX
“H”, Appendix A-Documentary Requirements for Alternative Methods of Procurement, the following
