Bones
The elbow joint is composed of three bones: the humerus, the radius, and the ulna (Figure 23-
1). The distal end of the humerus forms two articulating condyles. The lateral condyle is the
capitulum, and the medial condyle is the trochlea. The convex capitulum articulates with the
concave head of the radius. The trochlea, which is spool-shaped, fits into an articulating
groove, the semilunar notch, which is provided by the ulna between the olecranon and
coronoid processes. Above each condyle is a projection called the epicondyle. The structural
design of the elbow joint permits flexion and extension through the articulation of the
trochlea with the semilunar notch of the ulna. Forearm pronation and supination are made
possible because the head of the radius rotates against the capitulum freely without any bone
limitations.
Articulations
The elbow complex consists of three separate joints: the humeroulnar joint, the humeroradial
joint, and the proximal radioulnar joint (Figure 23-2). The humeroulnar joint is the
articulation between the distal humerus medially and the proximal ulna. When the elbow is in
flexion, the ulna slides forward until the coronoid process of the ulna stops in the floor of the
coronoid fossa of the humerus. In extension, the ulna will slide backwards until the olecranon
process of the ulna makes contact with the olecranon fossa of the humerus posteriorly. The
humeroradial joint is the articulation of the lateral distal humerus and the proximal radius. In
flexion, the radius is in contact with the radial fossa of the distal humerus, whereas, in
extension, the radius and the humerus are not in contact. The proximal radioulnar joint is the
articulation between the radial notch of the proximal lateral aspect of the ulna, the radial
head, and the capitulum of the distal humerus. The proximal and distal radioulnar joints are
important in supination and pronation. The proximal and distal aspects of this joint cannot
function without each other.
Elbow Musculature
The elbow flexors are the biceps brachii, brachialis, and brachioradialis muscles (Figure 23-
3). The biceps brachi originates via two heads proximally at the shoulder: the long head from
the supraglenoid tuberosity of the scapula and the short head from the coracoid process of the
scapula. The insertion is from a common tendon at the radial tuberosity. The biceps brachii
flexes the elbow and supinates the forearm. The brachialis originates from the lower two-
thirds of the anterior humerus and inserts on the coronoid process and the tuberosity of the
ulna. It flexes the elbow. The brachioradialis, which originates from the lower two-thirds of
the lateral humerus and attaches to the lateral styloid process of the distal radius, functions as
an elbow flexor, pronator, and semisupinator.
The elbow extensors are the triceps brachii and the anconeus muscles. The triceps brachii
consists of long, medial, and lateral heads. The long head originates at the infraglenoid
tuberosity of the scapula. The lateral and medial heads originate at the posterior aspect of the
humerus. The insertion is via the common tendon posteriorly at the olecranon process. The
triceps and the anconeus muscle cause extension of the elbow complex. The pronator teres
(Figure 23-4C) at the elbow joint and pronator quadtratus at the distal aspect of the wrist (see
Figure 24-2A) cause pronation of the forearm. Supination is caused by a combined action of
the biceps brachii and the supinator muscle. With both supination and pronation, the radius
will move on the ulna.
Nerve Supply
Nerves stemming from the brachial plexus (C4-T2) innervate the muscles that control
movement around the elbow joint (Figure 23-4A). In the cubital fossa, these nerves become
the musculocutaneous, radial, ulnar, and median nerves (Table 23-1).
Blood Supply
The subclavian artery becomes the brachial artery, whose branches supply the elbow (Figure
23-4B). The deep brachial, ulnar recurrent, and radial recurrent arteries eventually
anastomose with the brachial to supply the radial and ulnar arteries of the forearm. The
medial cubital, basilic, cephalic, and brachial veins collectively drain blood from the elbow
region into the axillary vein (Figure 23-4C).
FUNCTIONAL ANATOMY
The anatomical arrangement of the elbow complex allows for flexion, extension, pronation,
and supination. The elbow has approximately 145 degrees of flexion and 90 degrees of both
supination and pronation. The bony limitations, ligamentous support, and muscular stability
at the elbow help protect it from overuse and traumatic injury. Because of the more distal
projection of the humerus medially, the elbow complex will demonstrate a carrying angle that
is an abducted position of the elbow in the anatomical position (Figure 23-5). The normal
carrying angle in females is 10 to 15 degrees and in males 5 to 10 degrees. In the athletic
environment, the elbow complex can be subjected to forces ranging from overhead throwing
activities to blunt trauma that can cause various injuries.
The elbow is a critical link in the kinetic chain of the upper extremity. Functionally, the
hand and shoulder need the elbow for normal function. The connection between multijoint
muscles that affect the elbow will work both proximally and distally in the upper extremity as
a whole.

Artikulasi

Otot Siku

23-4C) pada sendi siku dan prtator quadtratus pada aspek distal pergelangan
tangan (lihat Gambar 24-2A) menyebabkan pronasi lengan bawah. Supinasi
disebabkan oleh aksi gabungan dari biceps brachii dan otot supinator. Dengan
supinasi dan pronasi, jari-jari akan bergerak pada ulna.
Pasokan Saraf
Saraf yang berasal dari pleksus brakialis (C4-T2) menginervasi otot-otot yang
mengendalikan gerakan di sekitar sendi siku (Gambar 23-4A). Pada fossa cubiti,
saraf ini menjadi saraf muskulokutaneus, radial, ulnar, dan medianus (Tabel 23-1).
Suplai Darah
Arteri subklavia menjadi arteri brakialis, yang cabangnya memasok siku (Gambar
23-4B). Arteri brakialis dalam, berulang, dan radial berulang akhirnya anastomosis
dengan brakialis untuk memasok arteri radialis dan ulnaris lengan bawah. Vena
cubital, basilic, cephalic, dan brachialis medial secara kolektif mengalirkan darah dari
daerah siku ke vena aksila (Gambar 23-4C).
ANATOMI FUNGSIONAL
Susunan anatomi kompleks siku memungkinkan untuk fleksi, ekstensi, pronasi, dan
supinasi. Siku memiliki sekitar 145 derajat fleksi dan 90 derajat supinasi dan pronasi.
Keterbatasan tulang, dukungan ligamen, dan stabilitas otot pada siku membantu
