Gentlemen:

I hereby agree to participate in the bazaar of Ayala Alabang Village Garden Club to be held on October 17-18,
2009 from 8:00 a.m. to 8:00 p.m. at Cuenca Community Center, Ayala Alabang Village, Muntinlupa City.

TERMS & CONDITIONS:

1. Participation fee is Three Thousand Five Hundred Pesos (P 3,500.00) per stall for two (2) days (inclusive of 2
chairs & 1 table 3’ x 6’ without table cloth). Reservation fee of Two Thousand Pesos (P 2,000.00) must be paid
upon signing the contract (cash or dated check, 3 days after signing of the contract) and the balance is due
two weeks before the date of the bazaar (postdated check). NO REFUND of this RESERVATION FEE will
be made for any reason whatsoever.

2. Any Returned Check for any Bank Reason shall be penalized with P 300.00/check and FAILURE to pay the
amount of the Returned Check with penalty is considered CANCELED TRANSACTION.

3. Request for an electrical outlet or chair/s must be made upon signing the contract. Organizer will not
entertain any concessionaires who will request an outlet or chair/s on the day of the bazaar. An additional
fee per day will be charged to concessionaire who will request for the following,

4. The CONCESSIONAIRES are responsible in securing their own goods. The ORGANIZER (Ayala Alabang
Village Garden Club) will not be held liable/responsible for any losses/damages that may occur during the
Bazaar. In addition, the ORGANIZER IS NOT BOUND TO REFUND PAYMENTS in case of natural
calamities (typhoon, floods, earthquake, volcanic eruptions and any similar acts of GOD).

5. NO BOOTH SHARING. ONE TABLE, ONE CONCESSIONAIRE.

6. NO SELLING OF PIRATED CD’s, VCD’s & DVD’s (software movies & music), NO SELLING OF SHARP
OBJECTS such as knives, etc.

7. Goods will not be allowed in the alleyways and in front. Goods and items must be stored only within the
allotted area of 5’ x 7’.

8. All check/s should be issued in favor of IRISH BETH ONG - Account# 0339-0604-28 BPI New Alabang
Branch (Authorized Ayala Alabang Village Garden Club Organizer).
