AFFIDAVIT OF DISCREPANCY

I, PRESENTACION DELOS TRINOS PINAROC, of legal age, a widow, Filipino and

a resident of 22 Jose Rizal St. Talisay, Balanga City, Bataan, after having been duly sworn to in
accordance with law, hereby depose and state:

1. That my place of birth, as stated and registered in my NSO Certificate of Live Birth,
attached as “Annex A”, is Manila;

2. That sometime in 1976, to be able to comply with one of my enrolment requirements

in college, my late mother Patricia Banzon Delos Trinos, caused the late registration
of my birth certificate whereby she stated that Balanga City, Bataan is my birthplace;

3. That from then on, I have used Balanga City, Bataan as my place of birth as reflected
in the following personal and official documents:

a) Office Identification Cards attached as “Annex B”

b) Marriage Contract attached as “Annex C”
c) Philippine Passport attached as “Annex D”;

4. That I am undertaking to cause the correction of my place of birth appearing in my

NSO Certificate of Live Birth to reflect the place of birth stated in my above-
mentioned supporting documents, that is, Balanga City, Bataan; and

5. That I am executing this Affidavit to clear any discrepancy which may arise from
personal and official documents.

FURTHER AFFIANT SAYETH NONE.
