That I, RAMIR B. ROSARIO, of legal age, Filipino citizen, married to RAQUEL ROSARIO, Filipino
citizen, with postal address at 237 Baybay Polong, Binmaley, Pangasinan, Philippines by these
presents do hereby NAME, CONSTITUTE AND APPOINT my sister ROWENA R. MONTERSOSO,
Filipino citizen with residence and postal address at 237 Baybay Polong, Binmaley, Pangasinan,
Philippines, to be my true and lawful Attorney-in-Fact, for me and in my name, place and stead
to do and perform all of any of the following:
1. To process my Eternal Plans pension claims/benefits and to pick up my check which I am
requesting for a Lump Sum Maturity Value Benefit of P100,000.00

2. To deposit, encash, endorse, or negotiate the check or checks that may be issued by
Eternal Plans, Inc. in connection with the pension benefits.

HEREBY GIVING AND GRANTING unto my said attorney full powers and authority to do and
perform all or any acts requisite and/or necessary to carry into effect the foregoing authorities,
as fully to all intents and purpose as I might or could lawfully do, if personally present and
acting in person, and HEREBY RATIFYING AND CONFIRMING all that my said ATTORNEY-IN-FACT
may also do or cause to be done under and by virtue of these presents.
