CHAPTER I

THE PROBLEM AND ITS BACKGROUND

Introduction

According to Madeleine Leininger spirituality is a relationship with a supreme

being that directs one’s beliefs and practices. And with the diverse spirituality or religion

of the millions of people, there are situations where a person expects to receive mutual

respect on their different religious beliefs, and this also applies to the nursing practices.

Nagai-Jacobson and Burkhardt proposed spirituality as a cornerstone of holistic

nursing suggested that questions appropriate to exploring a patient’s spirituality might

include how the individual understands God and what things give joy and meaning.

According to royal college of nursing in 2011 Spirituality in nursing care is one of

the aspect that was often we forget sometimes nurses just focus on delivering the

physical, mental, social needs of the patient. Spiritual care is also important because it

was like serving yourself as a whole person. Giving a chance to patient to share their

thoughts and problems, providing therapeutic touch or communication that may lessen or

ease the heaviness of what they feel. Cure is not possible for many illnesses, but there is

always a room for hope for healing. Healing can be experienced as acceptance of illness

and peace with yourself.

Spiritual care is one of the core being taught in school, how nurses provide

spiritual care to different religion. The way they view spirituality and religion.

Let’s all together see how nurses provide spiritual care and the way they

implement it, do they even implement it. Five private hospitals in Baliwag, Bulacan are

the target host.

Statement of the problem

This study is conducted to give answer on how nurses implement spiritual nursing

practice.

Specifically the study aims to answer the following question:

1. What are the perceptions of nurses about spirituality?

2. What are the different spiritual care practices mostly perform by nurses?

3. What are the different spiritual care practices least perform by nurses?

4. How nurses perform spiritual care needs of the client?

Significance of the Study

This study determined the presence of spirituality in every nurses, how nurses can

improved way of giving care to patient adding spirituality on it.

Scope and Delimitation

This study focused on nurses of private hospitals, in Baliwag.

All private hospital within Baliwag vicinity are part of the study. This include the

following Castro Maternity Hospital and Medical Center, Marcelo Hospital, Rugay

General Hospital and Sagrada Familia Hospital.

Purpose of the Study

The purpose of the study are:

1. Determine their way of respecting patient spiritualty

2. Determine if nurses think that spirituality is one of the vital aspect needed in

implementing care to the client.

3. Distinguish different way of how nurses apply spirituality on their plan of care

Assumption of the Study

1. Nurses have different point of view when it comes to spirituality.

2. Nurses, being unique individual have their own different way of respecting

patient spirituality.

3. Nurses think that spirituality is vital in giving care.

Definition of Terms:

Merriam-Webster 2018

 Belief.an acceptance that a statement is true or that something exists.

 Care. a disquieted state of mixed uncertainty, apprehension, and responsibility.

Caring. Is showing concern and looking for those people who are not able to look

for themselves.

 Connection. The act of connecting: the state of being connected.

 Dimension. A level of existence or consciousness.

 Holistic. Relating to or concerned with wholes or with complete systems rather than

with the analysis of, treatment of, or dissection into parts.

 Holy. Exalted or worthy of complete devotion as one perfect in goodness and

righteousness.

 Implement. To use in the performance of a task.

 Influence. The power or capacity of causing an effect in indirect or intangible ways.

 Practice. To do or perform often, customarily, or habitually.

 Religion. A personal set or institutionalized system of religious attitudes, beliefs,

and practices.

 Spiritual. Of, relating to, consisting of, or affecting the spirit. (Merriam-

Webster,2018)

 Spirituality. Something that in ecclesiastical law belongs to the church or to a

cleric as such.

 Surrogate. To put in the place of another.

 Faith. Something that is believed especially with strong conviction involves.

 Spirituality and Nursing Practice. Spirituality and beliefs related to transcendence

or to nonmaterial forces of life and nature. Religious practice or religiosity relates

to a person’s beliefs and behaviors associated with a specific religious tradition or

dissemination.

 Spiritual Care. The aspect of health care that attends to spiritual and religious

needs brought on by an illness or injury. (Medical-Dictionary,2015)

 Private Hospital. Is a hospital owned by a for-profit company or a non-profit

company and privately funded through payment for medical services by patients

themselves, by insurers, governments through national health insurance programs,

or by foreign embassies. (Wikipedia, 2015)

CHAPTER II

REVIEW OF RELATED LITERATURE AND STUDIES

The researchers reviewed different literature and studies from sources or materials

such as books, journals, magazines and current issues from the internet were in it includes

learning factors that find the different aspect of how nurses implement spirituality.

Related Literature

In the Old Testament God reminded Moses that when he stood before the

Lord, He is on the holy ground. As a nursing student, nurse, clinical instructor, and every

nurses must all know that before we go to our clients and every staff in the hospital

settings we consider the hospital as a holy ground because this is where we serve our

client not just in physical aspect but on spiritual aspects as well, just like Moses before

God’s presence because taking off his sandals expressed an inward reverence through an

outward behavior in his worship. Just like when we go to other peoples house we took off

our sandals to show respect. In nursing profession nurses put themselves in other people’s

shoes. Nurses empathized with patients, pray for them, look out for their needs and we do

our best not just to heal them physically but they do their best to heal them in all aspects

especially in spiritual aspect. We considered hospital as a holy ground Nurses as God’s

surrogate and the extension of his healing hands even though nurses experienced personal

struggles, hardships, pain, language barriers and emotional burdens even though they feel

burn out in life. God is our healer as nurses heal others and when they feel that they are

not enough. God makes them feel that they are more than enough. Caring for clients is

considered holistic because nurses helps clients in every aspect. They are with them from

womb to tomb. Our Nurses role is to touch every little aspect in the patient lives as they

ask for God’s guidance and healing hands. (O’Brien, 2014).

In order to provide proper spiritual nursing care, the nurse should have enough

knowledge on it because it has a big impact on the patient. It influenced the health of

patient in connection to the effect of their spiritual belief. The interaction between the

nurse and the patient was affected by their opinion or their belief, having the same

opinion about it will help the nurse to gain the cooperation of the patient for them to

implement the practices or management that will help them to easily recover. (O’Brien,

2014).

Spirituality is believing in something whether you can see it or not, feel it or not.

It is the way or action that we do in accordance to our belief in our religion. It is faith that

you have or the one you praise whether it is a statue, Buddha, Monument, Rocks, Shrine,

Deities, Allah, or God. Showing your expression of gratitude to the one you worship

applying the command, pulse, principle on your daily life. Relating spirituality in nursing

care is a big scope aspect. Regardless of religion, nurses inside their soul the sense of

spirituality, apply in their care the wisdom, thought, worship of words, learnings, and

teachings. Applying spirituality in nursing care is also giving respect to your patient’s

spirituality and religion. Adapting to them and ensuring that no bounds will be build

regarding your belief in spirituality and religion. (O’Brien, 2014).

Spirituality in nursing practice is not just for the patients, but it is also for the

nurses because we all know that it is one of the aspect that can help nurses to provide

proper health needs, hopes for themselves ones how hard their work is and also for the

patients to cope up with their illnesses. That when people got illnesses they tend to

question our Creator, they are starting to lose hope and faith in God, and they are starting

to think that God is punishing them for any reasons. Sometimes they are asking why

them? That life is so unfair that does why the most important that nurses tend to do is

help them to regain their trust in God, help them not to deprive their spiritual needs.

Helping patients to think positive on life is one of the things that nurses can do, because it

will be the start to gain their spirituality and to establish again a tough faith on God,

helping them to realize that just because they have that illness is God punishing them and

help them to realize that all the things that they did before that leads them to their

condition. Sometimes having illness makes them realize and discover the people that

values them and make them feel that they are important. (Wattis, & Rogers, 2015).

Spirituality and religion has a big impact in Nursing Care that were providing on

patients, it is important on nursing care because it may help the patient to cope with their

illnesses. It may also improve the quality of nursing care because it may influence the

relationship of nurse to patient. Using spirituality in some other patient is important an

important thing or people in their lives, those who will help them and accept them even

they have that condition. It also makes the patient realized or learned on what reason that

cause them to have those illnesses, for them to change their way of thinking and accept

what state they have right now. Having hope in life helps the patient to recover or reduce

the factors that may worsen their condition. (Willemse, Smeets, Leeuwen, Janssen, &

Foudraine, 2017).

As we all know spirituality and spiritual care is one of the holistic nursing care

that is still overlooked because nurses tend to attend more on the physical needs of a

client. Spiritual care is the most important goal in nursing practice because it promotes

health, and reduces stress and anxiety, It also helps the mental health state of the client to

have a positive outlook in life. It helps the nurse to have a healthy state of mind,

strengthens faith, provides a quality of care to the patient not only on the physical aspect

but also in the spiritual aspect and I believe that spiritual care improves the client but

most importantly it improves the humanity and psychological needs of a nurse.

(Kharameh, Asayesh, Sharififard, &Alinoori, 2016).

Nursing is not only touching the concept of patient spiritual nature, but it also

relates vocation as well. Nursing is not just an art but it is a combination of art and pure

science. Nursing is a devotion that concerns in helping the patient not just physically,

emotionally, mentally but also spiritually. Nurses do holistic nursing care, because it

touched all the aspects not just only heals the physical body but it also heals the patient

heart and minds. (O’Brien, 2014)

In giving holistic nursing nurses need the body, mind, and spirit connection. The

body is a physical substance of a person where they perceived the spiritual care

interventions in the nursing practice, the mind is how they conceptualizes all the

dimension where they experienced reality and the spiritual connection is where we

connect with the highest being God. Holistic nursing is not complete without spiritual

connection because spiritual connection makes an individual to be truly healthy in all

aspects. When the nurse interacts with patients on a holistic level where they can care,

support, understand, love, empathized, consider and to rise above the limits the nurse is

standing on holy ground. (O’Brien, 2014)

As nurse stands on the holy ground on helping ill patients, God’s healing is the

most important thing. The nurses stand as God’s surrogate and the extension of his

healing hands nurses are healers because, they help restore the body of a patient not just

by giving them medications but also try to heal their faith and try to help the patient to

restore the body, mind and spirit the whole holistic being. Jesus is a healer he had healed

people just by touch and using physical materials to heal them. Being there for the patient

is a form of healing them and makes them feel that someone is there to listen and just by

giving his presence heals the patient. The nurses love and compassion for the people and

recognize the ability as a healer regardless of the social strata, background,

nonjudgmental attitude to the patient and being a compassionate healer just like Jesus.

(O’Brien, 2014)

Dimensions of Caring

In practicing spiritual caring there are three concepts in carrying out the

theological mandate in caring for the ill. Being with, listening and touching the patient. In

this act it can help the nurse to perform the spiritual care interventions in the nursing

practice.

Being

The notion of “being” means the nurse stayed with the patient who experienced

physical and spiritual pain, the experience of suffering, and other problems and support

them, makes them feel your presence in the time of crisis.

Listening

The notion of “listening” to their verbalization of anxieties and hardships of the

illness condition of the patient and making them feel that there is someone to listen and

listening provides spiritual care.

Touching

The notion of “Touching” nurses are the extension of God’s healing hands where the

nurse touches the patient not only physically but also touches the hearts and lives of the

patients by praying with them touches the spiritual relationship with God. (O’Brien,

2014)

According to the article authored by McBrien, “The importance of listening,

allowing the patient to speak and the provision of therapeutic touch are strategies

identified for honoring the spiritual needs of patients.” In this article it simply said

listening to the client and building rapport will help the client open up to you, because

giving spiritual care to the client who is uncooperative and no rapport, means that he/she

will not apply the spiritual nursing care to themselves and disregard everything that you

said and teach. They will not cooperate on improving their spiritual life and may not

addressed their spiritual needs. In order to gain the client’s trust and rapport we need to

give and take, give means we teach our teachings and take means accepting their beliefs,

respect their opinions and point of view. (Minton, Isaacson, Varilek, Stadick, &Persaud,

2017)

Textual Explanation

Nurses having the innate spirituality in themselves must know the other spiritual

care, according to -- there are different nurses role in giving spiritual care namely one’s

should know their own spirituality for them to know if others spiritual needs are being met.

Giving solution or resolving together one’s spiritual concerns, listening wholeheartedly, as

a nurse you should know your own limitation, knowing your patients spiritual resources

and expressed needs. Offering pray in times of need, being aware and showing respect to

patient’s culture, social and spiritual preferences, and recognizing the role played by their

religious leader, you can ask for their assistance in giving spiritual care.

CHAPTER III

RESEARCH METHODOLOGY AND DESIGN

This chapter presents the research design used in the study, respondents of the

study, research instrument, data gathering procedures and statistical treatment applied on

the study of the Spiritual Care Practices of Nurses in Private Hospitals in Baliwag,

Bulacan.

Research Design

The researchers used non-experimental descriptive designs which questionnaires

will be given to find answer and determine the prevailing conditions.

Research Environment and Respondents

The study was conducted at Castro Maternity Hospital & Medical Center, Rugay

Hospital, Marcelo Hospital and Sagrada Familia Hospital among the Nurses. The

researchers decided to include all of the nurses on those hospitals. Questionnaire is made

in a Likert’s scale type in order to determine the Spiritual care practices of Nurses in

Private Hospitals.

Research Instruments

The instrument used for data collection in this research study is a researchers’

made questionnaire. It has 37 questions which are composed of closed ended. The

questions are designed by the researchers to help identify the Spiritual Care Practices of

Nurses in Private Hospitals in Baliwag, Bulacan.

The questionnaires composed of four (4) parts. The first part reflected the profile

of respondents as to; Name (optional), Gender, Age, Religion, Hospital, Year Graduated

on Nursing, Years working in the hospital, School Graduated and Current Shift

Assignment. The second, third and fourth part consisted of statements in Likerts Scale

about Spiritual Index of Well Being Statement based from Daaleman, T. P. & Frey, B. B.

(2004), Spiritual Assessment Items based on Spiritual Assessment Scale by Jones and

Data Collection Procedure

A letter of seeking the approval of the conduct of the study was submitted to the

office of the Dean of the College of Nursing. After the dean has approved the letter,

another set of letter where distributed among the Chief Nurses of the Hospitals. After the

letter has been approved by Chief Nurses the researchers personally facilitated the

distribution of the questionnaires accompanied by a cover letter which explains the

purpose and objectives of the study, what will be done to the gathered information and

the confidentiality of the results. The researchers asked them to participate, distributed

the questionnaires and once the questionnaires were already filled up by the target

respondents. The researchers gathered the data and proceeded with the analysis.

Statistical Treatment of Data

Target hospitals for the approval for the conduct of the research.

The overall results of the questionnaires were presented using Tabular form.

Weighted mean and percentage were primarily used and describe the different activities

by the researchers and answer by the respondents. The formula of percentage includes:

CHAPTER IV

PRESENTATION, ANALYSIS, AND INTERPRETATION OF DATA

This study identified the Spiritual Care Practices of Nurses in Private Hospital in

Baliwag, Bulacan. The proper identification of this study will help to know how the

Nurses perform spiritual care in their clients. The study will provide important

information on the spiritual practices that still implement by the Nurses on their clients,

which is also one of the aspects of Nurses that they need to perform.

With this study, we will able to identify if the Nurses are still giving importance

in performing Spiritual Care.

The study is conducted primarily to: determine their way of respecting patient

spirituality, determine if nurses think that spirituality is one of the vital aspect needed in

implementing care to the client, and to distinguish different way of how nurses apply

spirituality on their plan of care.

The Part I of the table is composed of the personal information of the nurses, the

comparison on how many male nurses and female nurses works on four different private

hospitals, there age, years they had worked for their hospitals, their religion, and shift

assigned to them on current time.

As shown on table 1 there are more female nurses working in four different

private hospitals in Baliwag, Bulacan than male nurses. With female nurses of 80% out of

20% male nurses.

As shown on Table 2 there are more nurses aged 20 years old to 30 years old

having a 60% and 36.36% of nurses aged 31 to 40, while there are only 1.82% on both

nurses aged 41 to 50 and 51 to 60.

As shown on Table 3, nurses on four different hospitals had stayed from there for

almost 1 – 3 years having 49.09% which is 27 out of fifty 55 respondent comparing to

5.45% which is only 3 out of 55 respondent have stayed in for more than 10 years.

Followed by 10.91% which is 6 out of 55 respondent stayed for 5 – 10 years, 18.18%

which is 10 out of 55 respondent styed for 3 – 5 years and 16.36% which is 9 out of the

55 respondent stayed for less than a year in there hospital.

As shown on Table 4, there are no Jehovah’s Witness and Iglesia ni Cristo among

the respondent. There are 3.64% or 2 among the participants are Christian and the rest

which is 87.27% or 48 of the respondents are Roman Catholic.

As shown on Table 5 there are more respondents shifting from 7 – 3AM with

result of 49.09% or 27 out of 55 respondents, 29.09% or 16 out of the 55 respondent shift

from 3 – 11PM, and 10.91% or 6 out of the 55 respondents both shift from 11 – 7PM and

7AM – 7PM.

Part II answered the question in chapter one what are the nurse’s perception about

spirituality.

As shown on Table 2.1 in terms of having weighted mean of 2.25, nurses

disagreed that they are being overwhelmed when they are item number 1 having

difficulties or problems. Item number 2 having weighted mean of 1.86, tells that nurses

strongly disagreed that they can’t understand their own problems. Item number 3 with

weighted mean of 1.85 which means strongly disagreed therefore nurses have found their

purpose in life. Item number 4, according to the nurses, they always complete what they

have started for they strongly disagreed they don’t having this item a total weighted mean

of 1.83. Item number 5 with total weighted mean of 1.77 shows that nurses know how to

start solving their own problems in life. Item number 6 with total weighted mean of 1.74

hardship undergoing. Item number 3 with weighted mean of 2.22 tells that nurse’s

seldomly offer or pray with the clients. Item number 5 having a weighted mean of 2.17

means that nurse’s seldomly allow client to listen to a worship songs while admitted.

CHAPTER 5

SUMMARY, CONCLUSION, AND RECOMMENDATION

This chapter presents the summary of findings of the study, conclusion, and

recommendations as suggested by the researchers.

Summary

To summarize this the researchers made used of the questionnaires were

gathered and presented in the tabular form with their corresponding interpretation. From

the presentation it was revealed that there are more female nurses working in four

different private hospitals in Baliwag, Bulacan than male nurses. With female nurses

having 80% compare to male having 20%. The majority of the respondents belong to age

group of aged twenty years old to thirty years old having a 60% and 36.36% of nurses

aged thirty-one to forty, while there are only 1.82% on both nurses aged forty-one to fifty

and fifty-one to sixty. Nurses on four different hospitals had stayed from there for almost

1 – 3 years having 49.09% which is twenty-seven out of fifty-five respondent comparing

to 5.45% which is only three out of fifty-five respondents have stayed in for more than 10

years. Followed by 10.91% which is six out of fifty-five respondent stayed for 5 – 10

years, 18.18% which is ten out of fifty-five respondent stayed for 3 – 5 years and 16.36%

which is 9 out of the fifty-five respondent stayed for less than a year in their hospital.

There are no Jehovah’s Witness and Iglesia ni Cristo among the respondent. There are

3.64% or 2 among the participants are Christian and the rest which is 87.27% or 48 of the

respondents are Roman Catholic. there are more respondents shifting from 7 – 3AM with

result of 49.09% or twenty-seven out of fifty-five participants, 29.09% or sixteen out of

the fifty-five respondent shift from 3 – 11PM, and 10.91% or six out of the fifty-five

respondents both shift from 11 – 7PM and 7AM – 7PM.

As to their perception about spirituality on how they perceived your spiritual

quality of life the results revealed that having weighted mean of 2.25, nurses disagreed

Conclusion:

The researchers concluded that nurses from four different hospitals have the same

point of view regarding spirituality.

Most of the respondents have the same way of showing respect to patient’s

spirituality and they think that spirituality is vital in giving care.

Nurses know their spiritual aspect within themselves, and they know the worth of

patient’s religion and spirituality however, these are the spiritual practices that is seldom

done by the nurse’s. This is because of work overload, long duty hours and lack of staff.

In terms of how the nurses performed the spiritual care needs of the clients nurses

always stay for quite some time with patient experiencing pain, suffering and other

problems, and shows that nurses always listened to their client anxieties and hardship

undergoing all of this practices shows that the nurses perform the spiritual care needs of

the client.

The majority of the respondents are female, 20-30 years old, 20-30 years old, 1-3

years of working experience in the hospital, religion is roman catholic, and in 7-3 pm

shift.

The respondents strongly disagreed with the questions regarding on how they

perceived quality of life therefore they know how their perceptions regarding spirituality.

The respondents strongly agreed on the spiritual activities as an individual. They

also agreed that their There is a Supreme Being, I am at peace with God and I feel

confident that God is watching over me. the nurses showed that they implement their

spiritual activities.

The most performed spiritual care intervention of the client. The nurses always

stay for quite some time with patient experiencing pain, suffering and other problems,

and shows that nurses always listened to their client anxieties and hardship undergoing.

This shows that the nurses perform the spiritual care needs of the client and other

spiritual practices was seldomly performed by the nurses like discussing spiritual

resources around the community, offering prayer to clients, sharing inspirational writing

or words from bible and arranging visit or referral to spiritual mentors or priest.

Recommendations:

Based on the survey made the researchers would like to recommend that the nurse

must discussed with their client spiritual resources around the community to help in their

spiritual care needs, offer or pray with the clients, allow client to listen to a worship songs

while admitted, provide support in clients spiritual practices, encouraged clients to use

bible, seldomly arrange visit or referral to spiritual mentors or priest to help client in their

spiritual needs.
