1. Proposed Name of Corporation (give 3 proposed names in case the other

name is already taken by other entities)
2. Purpose(s)
3. Principal place of business (complete address)
4. Name of incorporators (at least 5 but not more than 15)
5. Nationality, and residential address of #4
6. Amount of capital contribution (minimum 5,000 pesos)
7. Tax Identification Number of each incorporator
8. Appointment of Corporate Secretary and Treasurer
9. List of members with residential address certified by the Corporate
Secretary
10. Calendar year or fiscal year? If fiscal year, date of commencement of
operations.
11. Date of annual regular meeting
