Thank you for your review and comments. This revision 3 has been prepared referring to the Engineer ’s letter
No.ENG/PONRE/X/2007/1128 of 14 Nov. 2007. Our responses to your comments mentioned in the referred
letter and major points revised are as follows.

1. Safety & Health Education

Prior to commencement of the gate shaft excavation works, safety and health rule will be established. Then
safety & health education and training for the shaft excavation will be carried out so that all the personnel who
will work inside the shaft will understand the rule.

2. Safety Gear

Safety gear for the shaft excavation will be prepared for all personnel who work inside the shaft. The list of
safety gear is shown below;

 Helmet
 Earplugs
 Eye Protector
 Reflector Vest
 Boots
 Dust Musk and Filter
 Gloves

Distribution of the safety gear will be recorded by the safety officer. The form of distribution record is shown in
the Table.1

3. Assignment, Authority and Duty of Tunnel Safety Officer

The assignment, authority and duty of tunnel safety officer is shown below;

 We will assign the tunnel safety officer for each shift of shaft excavation.

 Since the shaft excavation works are carried out in 2 shifts, we will assign 2 tunnel safety
officers.

 The tunnel safety officers must be familiar with the method and sequence of underground
excavation works, thus, we select 2 members from the shaft workers and assign them as tunnel
safety officers.

 The tunnel safety officers shall monitor the whole of shaft works from the 1 st stage of the ladder
of gate shaft basically.

 The tunnel safety officers will be given power:

to instruct the shaft workers to take necessary preventive measures to avoid


accidents,

to instruct the shaft workers to suspend the underground excavation works when he
deems it necessary.

 All the shaft workers must obey the instructions given by the tunnel safety officer. Tunnel safety
officer have the power to kick out the workers who break the safety rules, or the instructions
given by the tunnel safety officer from the underground excavation site. If the worker did not
obey the instruction given by the tunnel safety officer, we will issue the warning letter to him.

4. Safety Rule of Shaft Excavation.


4.1. General Safety Rule

 Workers who work inside the shaft shall always wear helmets, boots and reflector vests.

 Ventilation fan shall be operated continuously except while blasting and no working days.

 Tunnel supervisor shall always take a whistle with him.

 Shaft face and shaft support shall be checked everyday.

 Sufficient lighting shall be prepared in the work area.

 Electrical cable and panel shall be checked everyday.

 Tunnel supervisor and foreman supervises basically works at the position where the whole of
working situation can be seen.

 The daily check shall be carried out before using. Please refer to the check list in the sub section
9.

 Needless materials shall not be left inside the shaft and working place in the shaft shall be
always kept clean and neat.

 When abnormal condition is found in the shaft, inform the Resident Engineer immediately.

 Smoking during working at working place is strictly prohibited.

 First-aid kit shall be prepared at the worker’s hut near the initial concrete.

 Tunnel safety officer shall always take a whistle and a flashlight with him and he shall wear
special helmet indicating that he is the tunnel safety officer.

 When workers are working at the face of the shaft, it is strictly prohibited to carry out any
works above them.

 A small ditch of approximately 20cm wide and 10cm deep will be constructed around the initial
concrete by digging the ground to prevent rain water from coming into the shaft. The small
ditch will be always kept clean and tidy.
 Drinking water shall be always available near the working area for workers to drink and wash
eyes.
 The operation of the manual loading equipment shall be controlled by the tunnel safety officer’s
instruction. The tunnel safety officer will communicate with workers at the face and operators
of manual loading equipment by his whistle or orally.
 The tunnel safety officer shall conduct the crane operation by the signals of the crane operation
(Refer to Figure 2). Before the crane operation, he shall give the instruction, that workers should
stay away from the face, to the workers by his whistle or orally

4.2. Safety Rule for Drilling, Charging and Blasting Works

 Drilling and charging shall not be carried out at the same time.

 While drilling by using leg drills, the operators of leg drill shall wear dust musk and earplug.

 Sleeves of worker’s clothes shall be fit to their arms so as not to be rolled in the drilling
machine.

 Welding work inside the shaft shall be prohibited when explosives are in the shaft.

 After blasting, sufficient fresh air shall be supplied by the ventilation fan before the workers
enter the shaft.

 The details of safety measures for blasting are mentioned in the section 6.

4.3. Safety Rule for Mucking, Chipping and Scaling Works

 Before commencement of mucking, condition of hoisting bucket shall be checked.

 While mucking is carried out, nobody except an operator of the excavator is allowed to enter the
shaft. If required to enter, mucking shall be suspended and the excavator shall not be operated.

 The tunnel safety officer monitor the mucking works from the 1st stage of the ladder. He shall
prevent anybody from entering the shaft during mucking.

 Overload on the hoisting bucket is prohibited.

 While chipping and scaling is carried out, the workers inside the shaft shall wear the dust musk.

4.4. Safety Rule for Shotcrete Works

 While shotcrete works is carried out, the workers shall wear dust musk and the nozzle man shall
wear eye protector, too.

 If compressed air leaks from the air supply hose, the works shall be immediately suspended and
the air supply hose shall be repaired.

 During the shotcrete operation, whistle sound by the tunnel safety officer means the emergency
stop of shotcrete machine.

 During the shotcrete operation, the tunnel safety officer shall monitor the spraying condition. If
the conveyer hose of shotcrete is clogged with shotcrete material, he shall give the instruction to
the operator of shotcrete machine by his whistle, that shotcrete operation shall be immediately
stopped.

 The voice signals exchanged between the nozzle man and the operator of shotcrete machine will
be transmitted by the tunnel safety officer’s voice or whistle.

 When fast accelerator admixture gets in eyes, immediately wash eyes with drinking water.

 Before cleaning of the shotcrete machine is carried out, the power plug shall be pulled out from
the socket.

4.5. Safety Rule for Rock Bolt Works

 The worker who inject the mortar into the holes shall wear eye protector.

 When mortar gets in eyes, immediately wash eyes with drinking water.

 If mortar is stuck inside injection hose, mortar injection shall be immediately stopped.

4.6. Safety Rule for Steel Rib Support Works

 Steel rib support materials shall be transported to the shaft face by the crane

 Adjustment of the holes for the center bolt of the steel rib supports shall be carried out by using
a steel bar or a ratchet wrench. Do not put fingers in the hole of the center bolt.

4.7. Safety Rule for Loading Equipment, Materials and Tools onto the Face

 Before loading, the conditions of the hoisting tools shall be checked. The hoisting tools for each
loading is determined as follows;

 Before loading, the balance of loading will be confirmed.

 During loading of equipment and material by crane, nobody shall stand by inside shaft.

 During loading of tools and materials by the manual loading equipment, nobody shall stand by
at the face of the shaft, however, workers will be able to stand by at the stage of ladder.

 The sign of loading and unloading will be carried out by the tunnel safety officer only.

 When materials, tools and equipment reaches to the face, workers can go to the face in order to
remove hook and hoisting tools.

4.8. Safety Rule for Workers Accessing the Face

 The limit number of workers for the stage and ladder is 2 persons and 1 person respectively.
Over riding to the stage and ladder is strictly prohibited.

 When they find the damage of the stage and ladder, the damage shall be repaired immediately.

 The removable ladder shall be firmly fixed by steel wire after each blasting.

 When the crane is available, hoisting bucket can be used as the accessing mean to the face.
However, the limit number of workers for the hoisting bucket is 2 persons only.

5. Daily Pre-Working Meeting

Pre-working meeting will be held by the tunnel supervisor before commencement of the works of each shift.

 The planned work schedule in the shift will be instructed by the tunnel supervisor.

 Geological conditions at the shaft face will be informed by the tunnel supervisor.

 Conditions of construction equipment will be informed by the tunnel supervisor.

 Safety Instructions are given by a shaft worker. To avoid mannerism, safety instructions will be
given by rotation among the shaft workers.

The tunnel engineer will participate in the pre-working meeting and confirm whether the meeting is properly
held.

6. Prevention of Blasting Accident


6.1. Order Explosives

 Those who order explosives are only blaster whom JO assigned.

 The blaster must have the license for blasting.

 Order quantity of explosives shall be determined as per actual drilling condition for a blasting.

 Both of blaster and administrator for magazine confirm the amount of explosives delivered by
checking with the order slip.
6.2. Return Explosives

 All of remaining explosives after charging must be returned to the magazine immediately.
Should not stock the remaining explosives on the blasting area until next blasting.

6.3. Drilling

 Before drilling, loose rock shall be removed by scaling.

 Drilling angle and depth shall be basically based on design plan of drilling and charging. Before
commencement of drilling, drilling pattern shall be marked on the ground.

 Explosion might be occurred when the last blast hole is used for drilling and the drilling bit
punches residual explosives. Therefore, the last blast hole shall not be used for drilling.

 Before commencement of drilling, it shall be confirmed whether there is residual explosives at
the face. When the leg wire is found in the last holes, the electrical detonator shall be checked
whether it has blasted or not.

 Drilling holes shall be cleaned up by compressed air before charging .


6.4. Charging

 Charging is prohibited during drilling.

 It is prohibited to smoke and fire during charging.

 When explosives are put on the ground temporarily until charging, explosives shall not be put
under the slope and around drilling machine.

 Charging quantity shall be basically based on design plan of drilling and charging.

 Care shall be taken to handle explosives. Should not make an impact to explosives.

 Connections of leg wires for electrical detonator shall not be attached on the ground.

 Fire cable shall be separated from electricity cables.

 In order to prevent the explosion by the current leakage, the fire cable of the blasting machine
side shall be short circuit.

 Fire cable shall be set up on the dry area.

 Electrical detonators shall be connected in series.

 Sufficient stemming shall be backfilled to each charged hole.

 After completion of charging, the cabling shall be checked again by using a tester.
6.5. Blasting

 The blasting machine with sufficient capacity as the flow of a current enough for each electric
detonator shall be used. Battery condition of the blasting machine shall be checked before using.

 All persons and equipment shall evacuate from the blasting area to safety area before blasting.

 In the case of shaft blasting, it is necessary to evacuate from face at least 100m. However, actual
safety distance is subject to actual blast condition.

 Firing position shall be opposite side of the blasting direction.

 Blasting information shall be made clear. In the case of shaft blasting, the time for blasting can
not be fixed due to successive cycle works. Before blasting, blasting alarm must be carried out
shown below. Warning alarm for blasting may be subject to change depending on the actual
safety condition.

 Everybody shall escape from blasting area at least 10 minute before blasting.

 Nobody enter the blasting area within 5 minute after blasting,

 In the case of shaft blasting, the ventilation fan shall be operated after blasting in order to
remove blasting fumes. After discharging of the blasting fumes by the ventilation, entering the
shaft can be permitted.

 After blasting, blasting result shall be confirmed. If misfire occurs, re-blasting might be
required. If some of explosives can be found due to small misfire, such explosives must be
returned to the magazine houses.

 After blasting, the tunnel supervisor shall check the excavated slope near the gate shaft and the
concrete structure of the spillway (Refer to Table 7).

7. Prevention of Falling Accident & Falling Object Accident

Gate Shaft will be constructed in 28 meter deep from the ground level. If somebody falls from the ground level
onto the face or are hit by the falling objects, it must result in a serious accident.
Preventive measures to avoid falling accidents and falling object accidents are shown in the Table 3.

1. Aspirated dust easy stick to the Bronchi and Lungs:

We usually breathe much "powder particle" that is "dust" into the lungs under normal living conditions.
Aspirated dust sticks to the texture of respiration system such as, the nose - the bronchi - the fine bronchi - the
alveoli.

A graph attached hereunder shows the relation of composure ratio and dust particle size. This graph is showing
that the relatively large dust particle stick to the nose and the bronchi but the dust particle become smaller, it easy
to stick to the alveoli.

2. 1~ 5 micrometer size dust easy to concentrate to the Alveoli

The dust sticks to the bronchi and/or the alveoli does not detain continuously. Sketch attached hereunder shows
which organ of respiration system can remove hour big size dust. Stuck dust to the bronchi removes out by fine
cilia of the bronchi. Until the bronchi, more than 5 micrometer size dust can be removed out. Less than 1
micrometer size dust is also known to remove out by breathing out or immune defense system.

Therefore, 1 ~ 5 micrometer size dust is easy to concentrate to the alveoli.

3. Symptoms of Pneumoconiosis (Dust Disease)

In the case of much concentration of dust at the Alveoli, it usually observe that the respiratory function become
less. For example, even walking, it creates shortness of breath. If this symptom becomes heavy, then
complications can be easy to occur such as pneumonia in case of catch a cold only.

4. To Prevent Dust Disease

Dust protection mask distribute to all workers who work inside shaft. The mask is inevitably
necessary for the works inside shaft, such as shaft excavation, mucking, chipping and
shotcrete. If you use the mask correctly you can prevent disease caused by the various dust inside
shaft.

How to Use the Mask:


1. Check the mask before put on

Confirm the filter was not stained and


also check cleanness inside of the
mask.
Filter will be supplied in the case
necessary upon request by changing
with stained one.
If inside the mask was not cleaned, the
mask has no meaning to use.

2. Dust protection mask shall be fixed completely

Strap shall be suspended at


backside of helmet.

3. Put the mask on the face without any opening around

Check if any space left


between face skin and mask
edge.

Important point to use the Mask
1. Filter shall be set correctly according to the indication of direction.

The filter shall be set like


this: the arrow mark
direction become to mouth
side.
If put the Filter wrong
direction, the function will be
declined.

2. Filter shall never be dismantled and/or remodeled.


No other masks shall be used (Only distributed dust protect mask is allowed to use inside shaft)
