I, (BUYER), of legal age, Filipino and a resident of B5 L18 Buenavista St., Greatwoods
Highlands Brgy. Molino 3, City of Bacoor, Cavite hereby undertake that I am the purchaser a
real property particularly located at of B5 L18 Buenavista St., Greatwoods Highlands Brgy.
Molino 3, City of Bacoor Cavite;

That I am purchasing the said real property through Pag-Ibig Fund;

That pursuant to the Notice of Deficiency dated 27 December 2018, I was made to submit
another ID with clear signature of seller, S, however I could no longer comply with the same
considering that the latter could no longer be located or contacted or has not replied anymore to
our correspondence;

Likewise, as regards the required document on CONFORMITY LETTER duly executed

by the SELLER, the same could no longer be complied owing to aforesaid reasons;

Thus, I am appealing for your kind assistance on these matters in order for us to finally
settle or comply with your office’s requirements in order to transfer/ award the subject property
which we have been occupying / possess for the longest time now;

I am executing this affidavit to attest to the truth of the foregoing facts for whatever legal
intents and purposes it may serve.
