Dear Parents,


To better serve you, we are updating our school policies to include two new
provisions regarding teacher cancellations and payment methods. During the week
of March 3, please make sure to come into the school with the student to review and
sign the updated school policies. If you have any questions, please contact the

Cancellations:

The Jackson Massillon School of Music encourages our teachers to maintain
an active performance career, excel in their university studies, and to care for their
physical and mental health. As such, teachers will be permitted to cancel lessons no
more than 6 times per calendar year: two cancellations from September –
December, two cancellations from January – May, and two cancellations June-
August.
Within the bounds of these excused cancellations, makeup lessons will be
scheduled at your convenience. Beyond these two cancellations by the instructor,
parents will be offered the opportunity to either receive a credit on their next
months invoice or to schedule a makeup lesson.
We strongly encourage you to schedule makeup lessons for any missed
lesson by the teacher or the student, although we understand that this can be
difficult and inconvenient.

Payment, effective March 24, 2019 (April Tuition)

The Jackson Massillon School of Music will be switching over to online
payments only. Payments made through ACH will have no processing fee, however,
there will still be a 3.5% fee associated with all credit and debit card transactions.

You will have the option to enroll in auto-pay, where your account will be
charged the 24th of each month or to review invoices before your preferred payment
method is charged. We will process all payments on the first of each month.

Student Policies

______ I understand that I am registering for recurring weekly lessons at the Jackson Massillon
School of Music

______ I understand the Jackson Massillon School of Music charges a monthly tuition.

30 minute lesson 45 minute lesson 60 minute lesson
4 week tuition $100 $150 $195
5 week tuition $125 $187.50 $243.75

The following rates will apply for families enrolled in multiple lessons.

Scheduling

______ I understand that if I contact Jackson Massillon School of Music at least 24 hours prior to
my lesson, all efforts will be made to reschedule my lesson without penalty.

______ I understand that Jackson Massillon School of Music permits two day-of cancellations
(within 24 hours) per quarter, without penalty.

______ I understand that my instructor is permitted to cancel lessons twice from September-
December, twice from January-May, and twice from June-August , and that for these
cancellations a makeup lesson will be scheduled.

______ I understand that if a substitute is provided, it does not count against a teacher
cancellation.

______ I understand that beyond these two teacher cancellations, I may request either a credit on
my next month’s tuition or to have a makeup lesson scheduled.

______ I understand that if I cancel a lesson, no refunds or credits are given, but a make-up
lesson is scheduled.

______ I understand that if I need to cancel a lesson, I must do so by email or phone/voicemail.
Failure to contact the office in the event of cancellation will be considered a “no show” and no
make-up will be given.

______ I understand that make-up lessons must be scheduled within thirty days of the
cancellation. If I do not schedule a make-up lesson within thirty days, I will forfeit that lesson.

____ I understand that in the event of a planned absence, I understand that I am responsible to
pay for those lessons, and that a makeup lesson will be scheduled.

Payments

______ I understand that Jackson Massillon School of Music only accepts payment online through
ACH and Debit/Credit card transactions and that my tuition is due by the first of each month.

______ I understand that Jackson Massillon School of Music will send out my monthly invoice on
the 24th of each month.

______ I understand that if I am enrolled in auto-pay, my preferred payment method will be
charged on the 24th of each month.

______ I understand that my preferred payment method will be debited on the 1st of each month,
if I have not already paid my monthly tuition.

______ I understand that the Jackson Massillon School of Music must receive my first month’s
tuition, whether it is a partial or full month, before I can begin lessons.

______ I understand there will be a $30 fee for returned payments.

______ I understand that ACH payments will have no processing fee

______ I understand that credit/debit cards have a processing fee of 3.5%

Withdrawal

______ I understand that If I want to withdraw from lessons, I must give the Jackson Massillon
School of Music two weeks written notice of my withdrawal

______ I understand that I am responsible for paying for the remaining lessons and/or balance
due on my account.

______ I understand there is no temporary withdrawal.

I agree to abide by the policies and procedures of the Jackson Massillon School of Music .
