SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THIS PRESENTS:

1. That I, ____ (name of sister of CARMEN), Filipino, of legal age, residing at _________ ADDRESS was
employed from Philippine Amusement and Gaming Corporation (PAGCOR) from March 2005 to June
2009;

2. That during said employment, PAGCOR grants provident fund to its qualified employees as a benefit in
case of retirement and/or separation from service/employment;

3. That as a former qualified employee of PAGCOR, I have accumulated fund from the Provident Office of
the PAGCOR;

4. That a check, issued by PAGCOR thru its Provident Office, payable to me as my provident fund refund
is already available for pick-up;

5. That in view of the fact that I cannot visit PAGCOR office to receive said check because of the work
load and the time demand by my master studies, I have authorized MR. CONRAD V. BADRE, a
PAGCOR employee, former officemate and good friend of mine, to be my true and lawful attorney-in-
fact, for me and in my name, place and stead, to represent me and giving him full power specifically to
receive the said check, in accordance with law.
