I have read the "Instructions" given on the reverse of the form and in the prospectus and wish that my son / ward be registered to take the Aptitude Analysis for admission to Mayo
College. I note that the acceptance of the registration form and fee will not involve the School in any obligation to admit my son / ward and that the registration fee is not refundable.
I undertake to abide by all the School rules and regulations for admission as in force from time to time. I also note that my son / ward will be permitted to sit for the Aptitude Analysis for
admission in the class desired by me, provided he is eligible for it on the basis of his class of study in his previous School, and his age. I accept that the admission can be accepted or

 IMPORTANT INSTRUCTIONS
1. Address for communication must be written clearly. Any change in address must be intimated to the Director. All information
regarding the Aptitude Analysis will be sent to the latest address registered with the College.
2. The Aptitude Analysis will be held on the 3rd Sat of Nov every year.
3. Any request regarding change in date of birth shall not be entertained.
4. Eligibility: At the time of Aptitude Analysis, for a particular class, the boy must be studying in :-
(a) One Class Lower.
(b) He must strictly meet the age requirements.
5. Permission to sit in the Aptitude Analysis may be withheld if the boy is not eligible as para 4 above or para 6 below.

7. The School reserves the right to refuse permission to boys to sit for an Aptitude Analysis without giving any reason thereof.
8. Registration fee is non-refundable.
9. Acceptance of the registration form and fee is no guarantee that the boy will be admitted and it does not involve the school in any
obligation to admit the boy.
10. This form together with
i) Copy of Birth Certificate, Copy of Aadhar Card, Copy of Passport (if available)
ii) Letters of reference (not necessarily related to Mayo College)
iii) A bank draft as mentioned in the fee leaflet, drawn in favour of MAYO COLLEGE, AJMER payable at Ajmer should be sent as early
as possible.
11. All correspondence regarding admission should be addressed to the Director, Mayo College, Ajmer - 305001 to receive his prompt
attention in a cover superscribed 'Admission' on the left hand corner.
