Know Your Network. Your plan has its own provider network, a select group of doctors, clinics, hospitals and pharmacies that
have agreed to provide a full range of covered health care services for your plan. To get the most of your benefits, it’s important
to use providers in your plan’s network. You will have to pay more if you use a provider who isn’t in network. If you have an HMO,
you may even have to pay the full amount. Using a hospital out of your network could mean you pay some or all of the
hospital bills.

The following hospitals are providers in the Blue Cross and Blue Shield of Illinois PPO, Blue Choice PPO and Blue Precision
HMO Hospital Networks. Hospitals that belong to a network have a dot in that network’s column. Hospitals are listed in
alphabetical order by city. If you require care when you are outside the local service area, you can find a BlueCard® participating
provider by calling 1-800-676-BLUE (2583).

Although there may be some change in your Hospital Network from time to time, selection of Participating Hospitals will
continue to be based on range of services, geographic location, and cost effectiveness of care. Please visit our online Provider
