PROCESS FLOW FOR DAILY CLINIC VISIT

Teacher receives medical complaint from

- Teacher fills up the form.

Clinic Services
Teacher accompanies the student to the Teacher
Step 2 Referral Sip
Clinic.
- Teacher is to stay at student's side at all times
to ensure safety.

Step 3 Occupational Health Nurse (OHN) receives


the student & fills up the Nurse's Notes.
Medical Illness
- Log student's time in. Logbook
- Assess student's medical history.

Occupational Health
Nurse
Occupational Health Nurse conducts Health
Assessment. Nurse's Notes

Step 4 - Takes vital signs (BP, PR, RR, T & pain scale).
- Notes medical complaints, signs & symptoms.

Give interventions to medical concern


a. Non - pharmacologic management Occupational Health
Nurse
- Includes touch, music, aromatherapy, etc.
- Observation for 30 - minutes. Nurse's Notes
- If with improvement, remark accordingly &
accompany back to the
classroom.

Step 5
b. Pharmacologic management
- Medications given per orem. Medicine
- If student has emergency or maintenance Dispensing Logbook
medications on hand, OHN verifies authenticity of & Nurse's Notes
the medication & administers it.
- Observation for 30 - minutes.
- If with improvement, remark accordingly & Nurse's Notes &
accompany back to the classroom. Clinic Services
Referral Slip

Reassess condition.
- If condition has no improvement, parents / legal Occupational Health
Step 6 guardians are then called upon. Nurse
- Sent Home Request is issued.

Refer students to physician / hospital.


Permit to Leave the
*- Wait for parents / legal guardians to fetch the Campus Slip Parents / Legal
student. guardian
Step 7 *- Provide Sent Home Request & present to Guard
- on - duty.
*- Inform the student's teacher, SAO & Principal. Occupational Health

- Present Medical Certificate to OHN before
