have marginally changed your name or have changed your name/ surname after marriage select Yes,
else select No.

Country (If Born Abroad) If you were not born in India, please enter the country where you were
born. If born before 15/08/1947 in a place now in Pakistan or Bangladesh, enter "Undivided India".

Gender Select Male, Female or Transgender to indicate your gender.

Marital Status Select one of the options to indicate your marital status Note: If you are legally
separated from your spouse, select Divorced. If you are living separately from your spouse, but are not

Citizenship of India by Select one option to indicate the kind of your citizenship. Birth: If you are a

Is applicant eligible for Non-ECR category? Select Yes, if you are eligible for Emigration Check Not
Required (ECNR), else select No.

Visible distinguishing mark Enter any visible distinguishing mark on your body.
