Summer OJT
May 16, 2017
This day was my first on-the-job-training in the Office of the Director for Instruction and
NBC 461 or also known by the majority as D.I ‘s office. To start my duty, I assisted in the
monitoring of attendance of the professors during the summer class. I also secured the
signature of the director on the completion forms in the office, and after that I compiled it.
Before I leave, I made sure that the office is clean and all the electrical appliances are
unplugged from the outlets.
May 17, 2017
I assisted ma’am Delarmente to find the files she needs to evaluate the professors that
are qualified for promotion. I respond immediately if she needs a supply of bond paper or clips,
and get it in the stock room. Meanwhile, the director gave me an instruction to separate the
memoranda, office orders, and receiving copies and arrange them chronologically.
May 18, 2017
I segregated the old files from the new one, and rearrange them chronologically. As I
segregate I noticed that the Xerox machine was not functioning well, I reported it to the director
and he told me to label the machine out of order.
May 19, 2017
On this day, I compiled all the completion forms that had the secured signature of the
director. I had shown it to sir Ully one of my supervisors and he directed me to the registrar’s
office so that the forms would be encoded. From there, ma’am Dina told me to come back for
an hour to get our copy. As I came back the forms are encoded, I brought it in the office and
compiled it according to program.
May 22, 2017
I arrived at the Director’s office at exactly 2:00pm, as I came ma’am Delarmente
requested to me to get folders in the stockroom, and find the file of a certain professor because
she needs it for the finalization of points for NBC Cycle 4.
May 24, 2017
On this day, I rearranged the NBC Cycle 4 folders retyped its label and I alphabetized
its order. Meanwhile, the director told me to give a letter to the office of the vice president.
May 25, 2017
Ma’am Delarmente gave a task to the on-the-job-trainees to rearrange the supplies in
the stock room, because there would be supplies that are to be transferred in it.
May 26, 2017
A technician had arrived in the office today to fix the Xerox machine, after he took a
half an hour in repairing the problem. Finally the Xerox machine was available to be used
again, I waited for 5 minutes before I did the task that ma’am Delarmente told me that was to
photocopy the blank pages of Personal Data Sheets (PDS).

I arrived in the office early on this day, because it was still early ma’am Delarmente told
the on-the-job-trainees to lessen the stock of papers in the office and transfer it to the stock
room.
June 1, 2017
I assisted to print the faculty load sheets and matrix of the professors, before the
semester starts.
June 2, 2017
Forwarded the list of enrolled students to the professors using the system in the office.
I assisted some of the professors to find the list of students per subject and schedule they had,
and printed it for them.
June 8, 2017
I compiled the completion forms, sorted it according to course. Every time that a
student came to visit in to the office to ask for a copy of his/her completion form, sir Ully was
the one who talked to them.
June 9, 2017
The Director instructed me to change the arrangements of the papers pinned on the
corkboard, he sent me to the stock room to get three large file holders. After that, he told me to
nail it on the corkboard proportionally occupying the space and label each file holder – the first
one is for memoranda, next is for special orders, and the last one would be for receiving copies
of letters and the like.
June 14, 2017
I assisted ma’am Delarmente to clarify her checklist of files for the Q.C.E Cycle 5 that
is in the office, hidden somewhere in the cabinets. I checked the cabinets one by one and
found out that her list was correct and accurate.
June 16, 2017
I compiled the completion forms that had the secured signature of the director, and
filed the new memorandum, office orders, and receiving copies and attached them on the file
holders on the corkboard.
June 23, 2017
I segregated the white papers that are stocked in the office and put them on a box or a
sack and brought it to the stock room. After that, I added stocks such as scotch tapes, folders,
and other supplies that the office staff would need for the incoming days.
June 28, 2017
I assisted the new community service volunteers in the office; I helped them in doing
some of their task. After that, I assisted ma’am Delarmente to look for the files she needed.
June 29, 2017
I arrived in the office early on this day, so I decided to clean the cubicle where the
director stayed. After I had finished cleaning, I checked the equipments of the office if it was still
functioning well such as the printers, PC, Xerox machine, and etc.
July 6, 2017
I assisted ma’am Delarmente our NBC coordinator to photocopy the files of ma’am
Polison and some blank PDS.
July 7, 2017
I assisted the director to create a new school directory, he gave me instructions in how
should I do the task.
July 10, 2017
The director had given me a task to print the faculty load sheets and faculty matrix of
the professors.
July 12, 2017
I compiled new files, memoranda, and letters, including the letter of promotion for the
professors.
July 13, 2017
I distributed the faculty load sheets together with the list of students from which I had
given from the different chairpersons under the General Education Department.
July 14, 2017
I photocopied the papers needed for Q.C.E, and the director assigned me to ask the
professors under the General Education Department about their official time and designations
so that the honorarium budget could be made.
July 18, 2017
The director had given me a task to strategically locate all the professors in EARIST;
first, I must secure the signature in the faculty load sheet of those professors who had the
General Education subject loads and while I secure their signature they should fill-up the
information about their official time and designations; moreover, for those professor who had no
General Education subjects I should collect some information about their backgrounds in
studying because it would be used to create a new school directory.
July 20, 2017
Before I continued my task, I reported to the director all the professors that I had
approached already yesterday. And because of it, I had met new professors from the different
colleges. After I reported, I continued my unfinished task.
July 21, 2017
I continued my task today, although if I was to compare the thickness of paper that I
carried from the past 2 days it was few now.
July 25, 2017
I am not yet done with my task, because there newly printed faculty load sheets given
to me. I have to continue my task today, after I had done my task ma’am Delarmente asked me
to photocopy 25 Personal Data Sheets (PDS) for cycle 5.
August 1, 2017
The director had prepared a team of on-the-job-trainees that would conduct the Q.C.E
Cycle 5 on every college. But, before the team start they must get a sample of evaluation forms
to ma’am Angel at College of Arts and Sciences (CAS). She gave the soft and hard copy of the
evaluation forms and the team started the first step, they printed the evaluation forms as many
as possible.
August 4, 2017
On this day, the team led by ma’am Delarmente who would conduct the Q.C.E
evaluation continuously printed the evaluation forms in the office.
August 7, 2017
The target number of evaluation forms needed to conduct the Q.C.E was not yet
enough. While waiting, ma’am Delarmente gave me a task to find the persons in the College of
Engineering (CEN) who had given their empty green folders in the office, now I must return
these folders with PDS and other papers needed for the documentation to them so that they
can fill it up.
August 8, 2017
The folders that I gave yesterday were back at the D.I ‘s office, and the signatures
were almost completed except the signature of the HR chief. So I brought the folder to the
Human Resource Department (HRD) and gave the folder to ma’am Christylane Bagabaldo the
chief officer of HRD.
August 10, 2017
Ma’am Delarmente had deployed the team to conduct the Q.C.E cycle 5 in the different
colleges at EARIST, Manila.
August 11, 2017
The Q.C.E cycle 5 evaluations was still in operation, I am assigned at the College of
Education (CED) to conduct the evaluation.
August 17, 2017
The team had gone to the CEN to conduct the Q.C.E. cycle 5 evaluation.
August 18, 2017
On this day I helped my co-on-the-job trainee to conduct the evaluation to the
remaining professors that was not evaluated yet. We started at the CED, and proceeded to
other colleges afterwards.
August 25, 2017
I assisted ma’am Delarmente in compiling all the Q.C.E cycle 5 folders that she had
done, I get a large box in the stock room and brought it in the office so that I can put the folders
inside it.
September 8, 2017
I transferred the Q.C.E cycle 3 folders in the stockroom wit my co-on-the-job-trainees.
And after that, I had gone back in the office to fill the empty space that cycle 3 had left with
cycle 4 folders and the same as cycle 5.
September 14, 2017
After I secured the signature of some professors, I gave the faculty contract for part-
timers to sir Ully.
September 15, 2017
Sir Ully gave me a task to secure the signature for the faculty load sheets and matrix. I
was assigned in CAS and CBA.
September 19, 2017
Ma’am Delarmente asked me to find the cycle 5 folders of some professors in CEN,
because she would check the content of the folder of this certain professor and how many
points could be added to him/her for the promotion.
September 22, 2017
Ma’am Delarmente requested to the on-the-job-trainees to reorganize the supplies in
the stock room, because something wrong happened. The water from an unknown leak had
gone directly to the stockroom and increased the risk to soak the supplies. The trainees helped
each other; the boys put the bond papers on higher surfaces like tables or chairs. While the
girls covered the files and other papers that could easily get wet.
September 26, 2017
I was given a task to create a table in EXCEL that shows the different schedules of the
General Education subjects on the different colleges, the rooms, and the number of students in
a room. This would be a guide for the director to see where and when to conduct the
departmental examination.
October 4, 2017
I printed my work and showed it to the director, he asked for some revisions. After I
revised my work I gave it back to him.
October 5, 2017
I had sent a reply letter from the director to the ISG office at exactly 5:00pm, due to a
complaint letter that had reached the Office of Director for Instruction with regards to the rules
of the school.
October 6, 2017
The director had given me a task to remind to all the professors under the General
Education Department about the final examinations, grades, completion forms, and other
important matters before the semester ends. Attached with this reminder was the IPCR that
was also in need to be signed.
October 9, 2017
I continued my unfinished task yesterday about the reminders and the IPCR. After that,
the director once again sent a letter to the ISG office.
October 10, 2017
I accompanied the new student assistants of the director to the places they should
know such as the stock room.
October 13, 2017
Ma’am Delarmente needed the files for cycle 6 of some professor in College of
Industrial Technology (CIT), she told me to search for it.
November 28, 2017
This is the first day of my OJT in Commission on Higher Education (CHED) – Central
Office. After the registration through biometrics, it was 11:30am already; the company assigned
me in OPRKM (Office of Planning, Research and Knowledge Management). To start my
training, my supervisor gave me some instruction for the task - and I started to consolidate the
customer feedback forms according to offices.
November 29, 2017
I arrived at the company at 1:00pm because I have attended a meeting in the school. I
continued my task yesterday, because there are still two boxes of feedback forms that I need to
sort. I separated the consolidated forms from the unconsolidated and labeled it.
December 1, 2017
I continued to sort out the customer feedback forms, because yesterday I almost
finished one and a half boxes of it.
December 4, 2017
I have arrived early in the office, as I came I immediately checked the consolidated
forms if it is still in the box and not jumbled. After that, I continued consolidating the remaining
forms that are not sorted yet. I finished sorting on this day and I prepared myself for next task
for tomorrow.
December 5, 2017
After I have done sorting the customer feedback forms, I created a tallied data on a
piece of paper for the categories Agree, Disagree, Don’t know, and etc. so that I can count the
number of persons who chose any of these categories; and transferred the data I gathered in a
database.
December 8, 2017
I continued encoding the data I have collected from the customer feedback forms and
sent my work to my supervisor as I have finished it. She made a short inspection to my work
and suggested few revisions, and after that I sent it again and I am done.
December 12, 2017
I was assigned for a new task, to collect all the supporting documents of the different
regions in the Philippines that were sent in the gmail account of planning division. But, because
one of my supervisors has a list or track of emails I used it to locate the supporting documents
so easily. Meanwhile, the chief of Planning and Resource Management Division told me to
encode a certain data in the supporting documents immediately such as programs monitored,
mediations, activities conducted, and HEI activities because these files are needed for
submission.
December 15, 2017
I continued the task I have begun yesterday; I inserted the new supporting documents
from Region IV-B to the database I have created in EXCEL format. After that, I sent the data to
ma’am Virgie one of my supervisors in planning division.
December 19, 2017
I borrowed the laptop computer from ma’am Luz one of my supervisors, and opened
the gmail account of planning division so that I can access the e-mails of supporting documents
from different regions in the Philippines.
December 20, 2017
I have borrowed the laptop computer to monitor the e-mails for supporting documents
of some regions of the Philippines who was not yet sending their documents until now. But,
because there are no e-mails yet – I decided to organize the first database I have created and
made the regions in consecutive order.
December 21, 2017
I borrowed the laptop and continued monitoring through gmail for the supporting
documents. But still, there were no supporting documents to compile. So, I decided to ask
ma’am Virgie a technique to convert PDF format files to EXCEL format – and she taught me a
very tactical way to do it. I did this so that if ever I am to encounter such problems I can already
do something about it.
December 22, 2017
I borrowed ma’am Luz’s laptop computer again to continue the monitoring for the e-
mails. But, sadly there is no supporting document to compile in this day.
December 27, 2017
I monitored for supporting documents to compile through gmail, but there is still no e-
mail for it. Instead, I reorganized the folder I created in the laptop computer and compile there
the entire task I have accomplished in my On-the-job-training so that I have it accessible if I
needed it.
December 28, 2017
I opened the gmail account of planning division to monitor the incoming supporting
documents for PCAR of different regions. But still, there were no supporting documents to
compile in this day. Instead, I asked for any task to be done to my supervisor while I have
nothing to compile yet.
December 29, 2017
I arrived early in this day, when I was to borrow the laptop computer from ma’am Luz I
noticed that she was not present. Ma’am Roselle suggested that I could use any desktop
computer in the office because some of my supervisors are absent in this day. I continued my
daily task on monitoring the e-mails of supporting documents.
January 8, 2018
I borrowed the laptop computer of ma’am Luz and began with my task, when I was to
open the gmail account of the planning division – one of my supervisors approached me, it was
ma’am Roselle. She said that Regions III and X had sent their supporting documents already. I
browsed it immediately and compiled the new data in my database and encoded some
information in it.
January 9, 2018
I arrived at exactly 8:36am in the office, upon my arrival I sat for a while and thought
about the number of regions that had sent their supporting documents already. I realized that
there are 9 regions that had sent already based on the database I have had. I reported it
immediately to ma’am Marilyn. Meanwhile, I continued monitoring for the remaining supporting
documents.
January 10, 2018
I continued the monitoring of the supporting documents in gmail account of the
planning division. While waiting for the e-mails, I edited the database and made it more
accessible.
January 11, 2018
I continued the monitoring of the supporting documents in gmail account of the
planning division. I sent the database that I had created to ma’am Virgie, because she needed
it already - but still there are regions that had not sent their supporting documents yet.
January 15, 2018
I arrived in the office at 8:37am in the morning, as I arrived I proceeded to do my daily
task to monitor e-mails. After an hour, ma’am Virgie gave me a new task to be done. She told
me to start compiling all other data inside the supporting documents such as special orders,
CAV, and other data except the one I had compiled already. Meanwhile, regions I, III, IV-A, X,
NCR, and V had sent their supporting document for the fourth quarter.
January 16, 2018
I continued the monitoring of the supporting documents in gmail account of the
planning division. As I monitor I had found that CARAGA region had sent their supporting
documents for PCAR already, I immediately compiled it in my database. After I finished
CARAGA region documents I browsed back all of the downloaded supporting documents from
region I and other preceding regions so that I can check if there are data that I had not
compiled yet in my database.
January 17, 2018
I continued to browse back regions III, VII, VIII, IX, X, and NIR (Negros Island Region)
one by one to collect the remaining data inside it. Coincidentally, region IV-B had sent their
supporting document and also CARAGA region that sent an updated version of it. I strategized
the way I compiled, I prioritized to compile first region IV-B and CARAGA. And after I finished I
continued browsing back.
January 18, 2018
Yesterday, ma’am Luz was absent that is why I used her desktop computer instead.
From there I compiled new data that I gathered and saved it in my flash drive. On this day
ma’am Luz was present so I borrowed her laptop computer and from there I transferred all the
data I stored in my flash drive yesterday. Furthermore, I continued to browse back the
remaining unchecked regions.
January 22, 2018
On this day, I had finished to compile the remaining data in every region that I have
browsed back. I made a few revisions on my database and when I had finished, it was almost
4:30pm in the afternoon so I decided to send it tomorrow instead.
January 23, 2018
I arrived at CHED at 11:26am in the morning, because of school matters that I had to
do. Although I am late, I had attended my training and did my task for the day that is to monitor
the e-mails for supporting documents. I have browsed a supporting document for PCAR in
gmail but it is in ZIP format and when I extracted the file, it was empty.
January 24, 2018
On this day, I sent the database I had created for the supporting documents to ma’am
Virgie. After that, ma’am Marilyn gave me a new task – to encode the Institutional Development
and Innovation Grantees 2016, and also the K-12 professional Advancement – STEM
Teacher’s training participants 2017.
January 25, 2018
After I finished encoding the file that was given to me, Ma’am Marilyn told me to count
the number of schools that participated in the said event. After reporting to her the number of
participants, she gave me another set of document to encode because the target number of
participant was not yet reached. So I continued encoding and finished it immediately; after that
she has told me to add a column Private/SUC on the EXCEL file that I am working with.
January 26, 2018
I continued monitoring the e-mails for supporting documents for PCAR on this day, and
after that I have browsed the newly sent supporting document of Region XI and inserted it to
my database.

I borrowed the laptop from my supervisor and monitored for the supporting documents
for PCAR using the Gmail account of planning division.

On this day I stayed for a half day in the office, I borrowed the laptop from my

planning division. I left the office at 12:00 in the afternoon.

February 22, 2018
On this day, ma’am Marilyn gave me a new task to encode the list of confirmed
attendees in the orientation regarding the Updating of SUC ’s PIP 2017-2022. She told me that
I could get the softcopy of the list in the gmail account of planning division. After I have
browsed the list, she gave me documents that consisted the personal information of the
attendees in the said event. I encoded the personal information such as gmail/email accounts
and contact numbers that was written in the document, and inserted it in the list.
February 26, 2018
I borrowed the laptop from my supervisor and monitored for the supporting documents
for PCAR using the Gmail account of planning division. Meanwhile, ma’am Teresa called me to
help ma’am Jobelle carry the compact discs and flash drives and brought it to someone in the
second floor.

I borrowed the laptop from my supervisor and monitored for the supporting documents
for PCAR using the Gmail account of planning division.
February 28, 2018
I borrowed the laptop from my supervisor and monitored for the supporting documents
for PCAR using the Gmail account of planning division.
March 1, 2018

I borrowed the laptop from my supervisor and monitored for the supporting documents
for PCAR using the Gmail account of planning division. Meanwhile, ma’am Marilyn told me to
create a dog-ear because she would be filing the 2017 Quarterly Accomplishment Report of
different offices such as AFMS, HEDF, OCC, OPSD, OSDS, OPRKM, UINFAST, PCARI, K-12,
LLS, OED, and OIQAG.

This is the last day of my duty at CHED; ma’am Marilyn gave me a task to photocopy
the Monthly Accomplishment of the different offices that is compiled in a folder together with the
other files. I carefully separated the needed papers as I photocopied it and returned to where it
should be.

Lesson Learned/Realizations
Summer OJT
I may have enjoyed the tasks that were given to me, but not all the times I made it
right. I admit that I could fail also - failed to reach deadlines, to do what was supposed to be my
task, and to give initiative and priority to the task given to me. I just realized that when a task
was already given to you, learn to say no if someone would be assigning you another tasks.
Especially if you know that the task given to you was far more important than any other tasks.
Another thing was about punctuality, always come earlier before the given time so you would
not be late.

Second Semester
Simplicity is beauty; I am not talking about the physical entity of a person. But it was
all about how I present the data in EXCEL. I tend to create data with a colorful scheme
because it looks presentable and creative I forgot that there were data that was not supposed
to need any colorful effects anymore such as the formal documents to be passed in a certain
office. Another thing was, lack of initiative because of being shy. I learned that I could never
learn – unless I tried.
