PETITION
Petitioner, by the undersigned counsel, to this Honorable Court, most
respectfully alleges that:

1. That petitioner is of legal age, Filipino and a resident of #321 San

Miguel, Mexico, Pampanga, where she may be served with notice and other
processes of this Honorable Court;

2. Respondent the Local Civil Registrar of Mexico, Pampanga may be

served with summons and other processes of this Honorable Court at the
Municipal Hall, Mexico, Pampanga. On the other hand, respondents the
Civil Registrar and the National Census and Statistics Office are government
instrumentalities impleaded herein in compliance with law, and maybe (sic)
served summons at the NSO Building, East Avenue, Quezon City;

3. That petitioner was born on December 23, 1969 at Mexico,

Pampanga to Sps. Antonio Sison and Natividad Aguas. However, when
Petitioner obtained a copy of her Certificate of Live Birth from the National
Census and Statistics Office, she found out that her name was mistakenly

and erroneously entered as Antonieta Dimalanta Aguas, copy of her

Certificate of Live Birth is hereto attached as Annex “A”.

4. Petitioner's true and real name is Antonieta Aguas Sison, and she
has used this name in all her scholastic and employment records, as well as
in all of her public and private transactions.

5. That this erroneous entry brought forth confusion to the record of

birth of the petitioner and it is now her desire to correct said errors so that
her true and correct name be reflected in her record of birth before the
respondent.

6. Likewise, Petitioner has secured pertinent clearances from the

Philippine National Police, The Office of the City Prosecutor, The Office
of the Regional Trial Court Clerk of Court, and The Office of the
Municipal Trial Court Clerk of Court showing that she has no civil,
criminal, administrative or any other case pending against her. Copies of
said clearances are hereto attached as ANNEXES “C”, “D”, “E” and “F”,
respectively.

To further prove that she has indeed no pending cases in the whole
country, attached hereto as Annex “G” is a copy of a Record Clearance
from the National Bureau of Investigation.

7. In order put a stop on the humiliation and embarrassment caused to

her by her name, Petitioner files this petition without any fraudulent purpose
or ill intent;

PRAYER
WHEREFORE, it is respectfully prayed that the Local Civil Registrar
of Mexico, Pampanga and it is now her desire to change the name of
petitioner from Antonieta Dimalanta Aguas to ANTONIETA AGUAS
SISON.
Petitioner also prays for other reliefs as may be deemed just and
equitable in the premises.

resident of #321 San Miguel, Mexico, Pampanga, state under oath that:

1. I am the petitioner in the above-entitled case;

2. I have caused this petition to be prepared. I have read and understood

its contents, including its annexes, and the same are true and correct to the
best of my knowledge and/or based on authentic records.

3. That I have not commenced any action of proceeding involving the

same issue in the any other court, tribunal or agency; that to the best of my
knowledge, no such action or proceeding, involving the same parties and the
same facts and issues, is pending in the any court, tribunal or agency, and
that, if I should learn thereafter that a similar action or proceeding has been
filed or is pending before these courts, tribunal or agency, I undertake to
report that fact to this Honorable Court within five (5) days therefrom.
