1. CONTRACTOR will perform its services to OSPI CARMONA in compliance with all applicable laws,
rules, and regulations, including, but not limited to, Philippine laws such ON Semiconductor’s Code of
Business Conduct and as Labor Code of the Philippines, Occupational Health and Safety Standards.
2. CONTRACTOR will ensure that its employees, agents, and invitees while at OSPI CARMONA premises,
comply with all applicable laws, rules and regulations.
3. CONTRACTOR must comply with OSPI CARMONA’s Corporate Social Responsibility (CSR) requirements,
including but not limited to Responsible Business Alliance (RBA, formerly EICC), Environmental,
Occupational Health and Safety requirements and Safety Culture initiatives.
4. CONTRACTOR will take full responsibility for the safety of its employees, invitees or any person who
enter OSPI CARMONA premise for any purpose related to the fulfillment of its obligations.
5. CONTRACTOR must take all reasonable measures and shall comply with OSPI CARMONA’s work and
special EHS permits as needed to prevent injury of any person or damage to any property within OSPI
CARMONA or in the vicinity thereof.
6. OSPI CARMONA shall be free from liability and CONTRACTOR shall indemnify OSPI CARMONA and
hold OSPI CARMONA, its affiliates and agents free and harmless from any suit or claim arising from
CONTRACTOR breach of performance with its obligations and warranties. OSPI CARMONA shall not
be responsible for any accident, injury or death that may be sustained by CONTRACTOR employees,
invitees or any personnel in connection with their services during their tour of duties, it being
7. CONTRACTOR will allow OSPI CARMONA to conduct inspections and/or audits as part of the latter’s
CSR and/or EHS requirements.
8. Without prejudice to any other remedies either Party may have under this Agreement or at law, OSPI
CARMONA shall have the right to immediately terminate this Agreement by written notice to the
Contractor if the Contractor fails to observe and/or to perform any of its obligations under this
Agreement (including but not limited to failure to comply with the aforesaid clauses, the safety
standards, Code of Business Conduct and all OSPI CARMONA policies), and where such failure is
requested by OSPI CARMONA to be rectified, does not rectify the failure within seven (7) days of
written notice from OSPI CARMONA.
SECURITY
1. Wear Security – issue ID at all times. (Contractor’s ID)
2. Wear contractors or companies uniform at all times inside the plant.
3. No Loitering and roaming around.
4. All materials and tools should be covered with gate pass. Work should not proceed without
proper tools, including ladders, fire extinguishers (for hot works) and other items necessary to
execute their job.
5. No alcoholic liquor, deadly weapon, illegal drugs and pornographic materials are allowed.
6. No shouting or use of profane languages while inside ON premises especially inside production
floor.
7. Extend courtesy to ON Semiconductor personnel.
8. Work permit should be signed by security OIC. In-case of overtime of the contractor. They
should have informed the project in-charge, facilities Operations Group and security OIC.
A. SAFETY
1. Comply with EHS procedures and permit requirements stated.
2. Follow fire and safety procedure set by ON Semiconductor, hot work permit systems, fire
watches during and after hot works jobs, lock-out / tag-out system, and emergency building
evacuations.
3. Eating at canteen is not allowed unless authorized by ON Semiconductor personnel in charge of
the project (Project Group only). Cooking of food inside the plant premises is also not allowed.
4. Eating must be done only in the designated time only.
5. Smoke only in designated smoking area.
6. Wearing of protective and safety gadgets whenever necessary. Wearing of safety shoes must
be provided when services and/or delivery of goods/items requiring the use of lifting device.
7. Do not proceed with the electrical work unless accompanied by a regular Facilities Technician
(applies in opening and terminating in the breaker side).
8. Contractors electrical tools should be provided with standard plugs and/or sockets specified by
ON Semiconductor
9. Do not set fire to debris or trash. Work with open flame should be covered with hot work
permit.
B. HOUSEKEEPING
1. Dispose debris/ waste properly.
2. Do restoration works in the affected area. (Ex. Paint jobs, wall partitioning, ceiling alignment
etc.)
3. Store materials and tools properly. Contractors should provide their own lockers or toolboxes
with locks.
4. Limited use of comfort rooms. Use only designated or assigned comfort room.
5. Wearing of prescribe apparel on specified area (ex. Smocks, ESD shoes, coverall suits inside
production floor)
D. PRODUCTION FLOOR
1. Use anti-static cloth as a partition separating Production from the contractors work area.
2. Contractor must use/ put-up a safety signage’s or caution tape as a safety pre-caution inside the
production area.
3. Contractor must wear a clean anti-static smock with hood or hat and a clean anti-static shoes
4. Contractor must be aware of the on-going productions and must always consult the production
supervisor before doing the work order in their area.
5. All tools must be at specification and with grounding.
6. No hot work activities inside the production floor unless with permission from Facilities and EHS
Managers.
7. No opening of ceiling board on top of machine.
1. Log-in works done at Facilities workshop and passing of activities for the day.
2. Seek facilities/project in charge approval of work done.
3. Surrender copy of special permits to project in charge if necessary.
4. Undergo usual Security Inspection.
