James Prochaska, a renown psychologist who
worked with alcoholics developed a model for
change known as the Transtheoritcal Model.

In this model for change he posits that change does


not happen instantly but rather happens through 5
recognizable stages that have their own sets of
challenges and priorities.

The 5 stages are as diagrammed: Precontemplation,


Contemplation, Preparation, Action, and
Maintenance.

When you mentally recognize and start to consider

that there is a problem but you are you are
noncommittal towards taking action, you are in the
“Contemplation” stage.

The key priority to move out of the Contemplation stage is to have crystal clear clarity
on the harms and costs of maintaining the status quo and to begin entertaining the
benefits of making a change and taking action.

These six questions listed if answered truthfully and thoughtfully will help you do that
emotional and spiritual evaluation.

There’s no need to judge yourself anymore for not “having it together” or “not changing
when you should have”.

The goal of this worksheet is to help you eliminate the “shoulds" and start to engage
yourself with compassion - so let’s dive right into the questions!
