D. Discharge Planning and Health Teaching

Medication

1. Inform patient about the timing and dosage of medications..

2. Educate patient and significant others about the expected outcome and

possible adverse effects of treatment.

3. Inform patient and significant others not to purchase drugs that were not

prescribed by the doctor to prevent complications.

Treatment

1. Let the patient and significant others know about the importance of following

the doctor’s prescription for the continuous recovery and promotion of

wellness of the patient.

2. Instruct patient to report signs and symptoms of infection including fever.

Exercise

1. A regular exercise is good to promote a healthier body to prevent from any

sickness.

2. Alternate rest and activity to conserve energy.

Hygiene and Comfort

1. Educate patient about the significance of hygiene:

a. Take a bath everyday to promote comfort and cleanliness.

b. Brush teeth at least 3 times a day or every after meal.

c. Cut nails in both fingers and toes.

d. Wash your hands often. Use soap and water. Wash your hands after

you use the bathroom, change a child's diapers, or sneeze. Wash your

hands before you prepare or eat food.

2. Get more rest and stay away from others.

3. Do not share food or drinks. Discard tissues after you use them to wipe or

blow your nose.

4. Identify signs and symptoms of an infectious process and report to the

physician promptly.

Out-Patient Appointment (OPD)

1. Inform patient and significant others about the importance of follow-up

check-ups to educate progress of treatment.

Diet and Nutrition

1. Ideally, meningitis patients should consume unrestricted quantities of fresh, organic

fruits and vegetables, moderate amounts of high-quality protein and generous portions

of healthy fats.

2. Meningitis patients are also advised to avoid eating foods loaded with salt, a primary

source of sodium

Spirituality

1. Foster patient and significant others about the importance of religion and

spirituality

2. Encourage patient and significant others to always pray for fast, continuous

recovery.

3. Inform patient and significant others to attend church services regularly.

4. Embolden patient and significant others to read the Bible for enlightenment.

Family Coping

1. Encourage patient to express of anger and concern about the limitations

that the disease and treatment impose, possible financial problems and job

insecurity.

2. Involve significant others in patient’s care: educate about stress

management techniques and maintenance of personal health for family

coping
