FROM JAPAN TO PHILIPPINE ON 4 JUNE 2012

SURVEY ONBOARD ON 13 & 14 JUNE 2012

1. ORDER

At the request of BAOVIET INSURANCE CORP and acting on behalf of the


owners and insurer of M/V “TRANSCO SKY”, the undersigned surveyor did,
on 13 & 14 June. 2012, attend onboard M/V “TRANSCO SKY” whilst she was
anchoring at Out of Port Limit (OPL), off Shanghai, China, for the purpose of
carrying out site survey on the captioned incident.

3. INVESTIGATION ON BOARD

Together with ship’s agent two service engineers, we attended on board M/V
“TRANSCO SKY” at Shanghai OPL by a tugboat on the later afternoon of 13
June 2012. We interviewed the ship’s master and chief engineer of the
vessel in ship’s office. Based on their presentations and the documents
available on board, we understood following information:

General Information

M/V “TRANSCO SKY” is a 15-year-old general cargo ship, designed with DWT
of 7833MT. The main machinery space, with accommodation above including
the navigation bridge is aft arranged.

Incident of the main engine

At about 1235 hours on 04 June 2012, while M/V “TRANCO SKY” was
underway from Tobata (Japan) to Manila (Philippine), the duty Engineer and
the oiler in engine room heard one abnormal “knocking” noise from the main
engine, and saw some smoke from the No.1 cylinder cover of the M/E. They
immediately reported to the Chief engineer and 2nd engineer, and then the C/E
decreased main engine revolution in order to avoiding further damages.

They cut the fuel oil supplying in No.1 cylinder of the main engine. About 1300
hours on 04 June 2012, they had to stop the main engine because of its high
temperature. They kept turning the M/E by the turning gear, and waited for the
temperature slowly dropping.

After the temperature was dropped, they checked and inspected thoroughly,
and found:
l No.1 main engine cylinder starting air valve was leaking;
l The combustion gases of No.1 M/E combustion chamber had blew into the
main starting air pipe, and further came into the starting air distributor;
l The starting air distributor was damaged by the combustion gases with
high temperature and high pressure.

Ship was towed from sea to Shanghai OPL

Due lack of spare parts of the air distributor onboard, ship’s crew could not
resume the running of M/E. After several days of drifting at sea, M/V
“TRANSCO SKY” was decided to be towed to Shanghai OPL for M/E
repairing.
The tug ship “Yun Tong 501” arrived at 1635 hours on 09 June 2012 and
commenced towing M/V “TRANSCO SKY” at 1820 on 09 June 2012. M/V
“TRANSCO SKY” finally arrived at Shanghai OPL and got anchored at 0030
on 11 June 2012.

4. SITE INSPECTION

Together with the vessel’s C/E, we inspected the main engine and found as
hereunder:

The engine crew had dismantled the damaged M/E No.1 cylinder starting air
valve and replaced it by a spare one. They also dismantled the starting air
distributor.

All the components of the starting air distributor that had been dismantled
were kept in the ship’s workshop. The damaged parts were:
l The driving shaft and the bushes bent;
l The driving gear broken;
l The bearing cracked;
l The turning disk deformed.

Meanwhile, we noted that the painting of main starting air pipe was peeled due
to the high temperature.

5. CAUSE OF DAMAGE

The chief engineer’s allegation

The main engine starting air valve leakage was the reason of the damage. As
the combustion gases with high temperature and high pressure came into the
main air pipe and the starting air distributor, the distributor damaged. Thus, the
main engine cannot be started.
Our opinion

Based on the damaged appearance, we concurred with the chief engineer’s


allegation regarding the possible cause of main engine breakdown.

6. REPAIRS

The two service engineers came on board with the spares of the starting air
distributor. They finished assembling the distributor at about 0400 on 14 June
2012. At 0500 hours on that day, the distributor was tested and the M/E was
started in satisfaction.

and the facts contained in this report are, to the best of our knowledge and
belief, TRUE AND CORRECT.

The Ship's under way From Tobata(Japan)to Manila (Phil), we've encountered
main enginehouble . About 12;35June04 / 2012 the watchkeepingEngineer
officer and Greasersaw the smoke at No1 Main engine cylinder cover and heard the
one "knocking" soundabnormally.My crew decreases main engineRevolutionand
reports to Wheel house ,Chief Engineer, 2/Engineer. We continue to decreaseM,/E
revolutiol in orderto preventseizureand sticking of anotherengineParts.And cut
fuel oil supplyingin Nol cylinder of M/E . About 13:00June04 I 2012 After report
Captain,we stop M/8. BecauseM/E Temperature's high , we perform to tum it by the
tuming gear and waiting for MIE temperaturedrop slowly to safety .
After check and inspectthoroughly, we found out and report as following : No I
main engine cylinder staxting air valve be leaked. The combustion gasesof No I M,/E
combustion chamber blew into the main starting air pipe and continue into the starting
air control valve Set (starting air distributor), with high temp. about 380-400 "C and
pressureabout 80 -100 kg/cm'. The combustiongasespressedto the tuming disk(No
6) of the startingair conhol valve - causedit be stoppedand causingdamageto engine
partsof distributoras following : - The driving shaft (No3) and bushes(No 8 & 9)
bent; - the driving gear (No5) broken 3 teeth; - the bearingsupport(No 2) be cracked
all round at position : close to the its fitting face with the main engine casing and
distance to fitting face about 10 mm ; - the tuming disk(No 6) may be deformed.
From the above mentioned damagesconsequence the main engine can not stad
by compressair . Due to level seriousdamages, lack sparcpartsof the startingair
distributoronboard - We might not rectify undership's on board conditions.
We reportcdto Ship owner 's technicaldepartment.My English may not be exact
to explain about to main engine failure. I give notice and reserving the right to extend
our ship's protest.
