AFFIDAVIT OF UNDERTAKING

I, IRENE N. TOLEDO, of legal age, Filipino, single and a resident of Brgy. Saraet,
Himamaylan City, Negros Occidental, Philippines, after being sworn to in accordance with law,
depose and state:

1. That I am the registered owner of a business name “RTJ Hardware Enterprises” located
at Purok 9, Brgy. Aguisan, Himamaylan City, Negros Occidental;

2. That I am applying for my business permit at the City of Himamaylan, Negros

Occidental;

3. That there is a controversy as to the real owner of the Lot which where the building of my
business is located;

4. That I hereby undertake to pay necessary taxes due to the City Government of
Himamaylan for the issuance of my business permit;

5. That I am executing this Affidavit to attest to the truth of the foregoing and for whatever
lawful purpose this may serve best.

IN WITNESS WHEREOF, I have hereunto set my hand this 18TH DAY OF January 2018 in
