As per the references cited above all the Medical Officers in the District are hereby
instructed to issue service certificates to ANMs/ MPHA(F) / 2nd ANMs who are
working on contract basis at concerned Institutions as per the guidelines issued by
the Commissioner of Health &Family Welfare, AP, Vijayawada,
Amaravathi. If any contract employee absconded from duties and breaking services
compulsory mention in the service certificate. The concerned DDOs are responsible
for verifying the service particulars of the contract employee. If any deviations, action
may be initiated as per the CCA rules. The service certificate must counter singed
by concerned Addl. DM&HOs/ Divisional Officers. The individual must submit her
first appointment order with service certificate to this Office.

In case of ASHAs, working certificate may be issued by the concerned Medical

officer and counter singed by the PODT of O/o DM&HO, EG, Kakinada including of
village health committee joining order, attested by the concerned Medical Officer.
The ASHAs who have completed MPHW(F) training course are eligible only.

The weightage marks : ( Contract ANMs, European ANMs, 2nd ANMs)

a) 1.5 marks for each completed 6 ( six months) for the contract service rendered in tribal
areas.
b) 1 (One) mark per each completed 6( six months) for the contract service rendered in
rural areas.
c) 0.75 marks per each completed 6 ( six months) for the contract service rendered in Urban
areas.
d) The contract service shall be calculated for every six completed months. There is no
weightage of marks for those even though they have completed the length of the 0.5
months and 29 days.
