*Ask the clerk’s monitor (CM) first if he’ll meet you first for the orientation or not prior to processing your
ID requirements.

Tasks
1) Clinics (M-Sat; see attached schedule of clinics of consultants)
Clinics are usually held M-Sat. The LO messages the clerks’ monitor to know if there are scheduled clinics
for the day. The CM tells the LO how many clerks should be assigned per consultant. The duty team is always
assigned to Dr. Noble. If possible, only assign duty clerks to clinics ending at 2 or 4 pm because it can usually
extend until the evening. Clerks are tasked to do history and neuro PE on new patients, observe during interactions
with old patients, and/or take the vital signs of patients (just ask the consultant what your task will be).
BRING: Stethoscope, neuro kit (particularly the neurohammer, tuning fork, otoscope, ophthalmoscope,
penlight), pen, booklet, 4 gray forms per consultant
NOTE: Dr. Noble’s clinic is at Room 251. Dr. Alemany usually just asks the clerk to take the BP of the
patient.
Don’t forget to let the consultant grade the clinic evaluation part of your booklet AND to
sign your four gray forms.
If the consultant asks you to accompany him/her during rounds, don’t forget to list the name
initials, age, sex, diagnosis of the in-patients that you’ve seen in your booklet.

2) OPD (TThF; 2nd floor, Tower 3)

OPD is done every Tuesday, Thursday, and Friday. Clerks are tasked to do history and neuro PE of both
old and new patients then refer them to the resident.
BRING: Stethoscope, neuro kit (particularly the neurohammer, tuning fork, otoscope, ophthalmoscope,
penlight), pen, booklet
NOTE: List the name initials, age, sex, diagnosis of the OPD charity patients that you’ve seen in your
booklet.

3) Duty
- In a nutshell: Duty team attends the endorsement rounds and doctor’s rounds, goes to the clinic with Dr.
Noble, and is part of the NRT who goes to the ER.
Get the number of your first year ROD. Text him/her to introduce yourself and to ask for further
instructions. You can either accompany the residents/consultants during rounds or have lectures/discussions with
them.
- Endorsement rounds (Neuro office, 3F, Tower 1)
Duty clerks must be at least 15 minutes early for the endorsements. Clerks just listen during the
endorsements.
M-F: 7:30 am, 4:00 pm
Sat: 7:30 am, 12 noon
Sun & Holidays: 8:00 am
- Doctor’s rounds (after endorsements; confirm with your 1st year ROD if you’ll have rounds ---
you’ll just accompany them/observe/do simple errands)
M-F: 4:00 pm
Sat: 12 noon
Sun & Holidays: 8:00 am
- When the speaker announces “Neuro response team/NRT to critical care bay area.” Go immediately
to the EMERGENCY ROOM, critical care area. Look for your resident then you just usually observe and follow
them there. If the NRT call happens during your rest break, you need not go downstairs anymore.

NOTE: List the name initials, age, sex, diagnosis of the in-patients that you’ve seen in your booklet.

4) Others
- Booklet: To be given by CM. Accomplish as much as you can. Attitude and professionalism part at the
last page is to be accomplished by CM.
- Clerks usually just stay at the quarters (10F, Tower 1, Male clerks’ room) when they don’t have
scheduled tasks for the day. The room can be cleaned everyday and beddings can be changed twice a week. Just tell
the housekeeping unit in the same floor (turn left, go outside the exit area, go past the doctor’s call room).
Servicemasters are only up until 9pm. Bring your own lock, toiletries, and snacks. Maintain cleanliness in the room.

- SGD, lecture, reporting, neuro demo/return demo: CM will inform the LO if the group will have one.
- Exit exam: Schedule to be confirmed with the CM.
- Last Sunday of the rotation: Everyone will assume preduty until 12 noon.
