MEMORANDUM OF AGREEMENT

This Memorandum of Agreement (MOA) is entered by and between SYDNEY HOTEL with
business address at Pendatun St., Pioneer Ave., General Santos City, hereinafter referred to as the HOST
TRAINING ESTABLISHMENT (HTE) and Mr. Jasper James A. Catubay, 20 years old, student of
Ramon Magsaysay Memorial Colleges hereinafter referred to as the STUDENT – TRAINEE and the
RAMON MAGSAYSAY MEMORIAL COLLEGES (RMMC), with business address at Pioneer
Avenue, General Santos City, hereinafter referred to as the HIGHER EDUCATIONAL INSTITUTION
(HEI) or SCHOOL do hereby stipulate and bind themselves as follows.

1. That the Host training Establishment:

a. Hereby accommodates the Student-Trainee and involve him/her in actual situations

preferably in the field of accounting and audit as OJT for 240 hours from__________ to
__________
b. Will not expose the Student-Trainee to hazardous activities;
c. Issue a Certificate of Completion in 3 copies to the Student-Trainee at the end of the
training;
d. Rates the Student-Trainee per RMMC’s Evaluation Report Form.

2. That the Student-Trainee:

a. Signs and comply with the previsions of the MOA at all times with the rules and regulations
of the HTE, CHED, and RMMC;
b. Completed the orientation and training program conducted by the school and the HTE;
c. Submits a monthly journals of practicum experiences describing the training activities,
problem/s encountered and a monthly Daily Time Records (DTR);
d. Signs the acceptance of the MOA, and submits duly notarized waiver;
e. Must comply the 240-hours OJT requirement;
f. Shall not demand salary and benefits while undergoing the on-the-job training;
g. Observes confidentiality with regard to all records, data, documents, company operations
and practices at all times and not disclose the same without the written consent of the
company;
h. Shall submit for review to immediate supervisor any paper or report regarding his/her OJT
and learning required by RMMC before submitting it to the BSA Director.

3. That the RMMC: College of Accountancy

a. Selects HTE and ensure acceptability of training manual and practicum venue in order to
protect the interest of the student
b. Provides and collects duly notarized waiver to the parents/guardians;
c. Evaluates the performance of the student-trainee jointly with the HTE based on the training
manual;
d. Monitors the student-trainee and attends to his/her needs and concerns by coordinating with
the HTE and other concerned government authorities if necessary;
e. Conducts general orientation for qualified student-trainees and their parents/guardians;
f. Issues a final grade to the student-trainee upon completion of the requirements on
prescribed period;
g. Issues a Certificate of Appreciation of the completion of training of students to the HTE;
and
h. Provides CHED Regional Office a copy of the duly notarized MOA.
4. That the HTE is not liable in case of accident that may result in injury or death or sickness of
the student-trainee, unless by the same are result of violation by the HTE of any provision of
this Agreement;

5. This document contains the entire Agreement of the parties with respect to the subject matter
hereof, and no other Agreement, statement of promise made by a party hereto, which is not
contained herein, shall be valid and binding;

6. That this Agreement takes effect on ____________ 2018 until _________ 2018 unless the HTE
may, upon written notice, pre-terminate the effectivity hereof.

IN WITNESS WHEREOF, the parties have thereto affixed their signatures at General
Santos City, Philippines, this ____ day of ______ 2018.
