HOUSEHOLD AND URBAN PESTICIDES

REQUIREMENTS FOR INITIAL PRODUCT REGISTRATION

1. Completely accomplished and notarized Integrated Application Form with ID picture of both the
Owner/Incorporator/General Manager and the Authorized Representative- c/o ECVPI
2. Notarized Application Form and Declaration – c/o ECVPI
3. Copy of Valid License to Operate (LTO) – c/o ECVPI
4. Duly authenticated by the Consulate of the Philippine Embassy from the country of origin of
any of the following:
 Certificate of Free Sale (CFS)
 Certificate of Good Manufacturing Practice (GMP)
 Manufacturing License
 ISO Certificate
5. Qualitative and Quantitative Composition of the Finished Product expressed in percent
6. Safety Data Sheet and Technical Specification of all the raw materials
7. Certificate of Analysis of the Finished Product duly signed and dated referring to the same
batch/lot number of the sample submitted; written/translated in English on the letterhead of
manufacturer shall include the following:
 Name and batch number of sample submitted
 Complete specifications, methods and limits stated on technical specifications of the
finished product
 All test results based on technical specifications of finished product stating actual
numerical value (when applicable)
8. Technical Specification of the Finished Product
 Appearance, color, state, odor
 Melting Point
 Boiling Point
 Vapor Pressure
 Density of Specific Gravity
 Solubility in water and solvents
 Suspensibility/Emulsifying Characteristics
 Known capability/incompatibility with other pesticide or active ingredients
 Flash point and other indications of flammability (If product contains combustible liquid)
 pH
9. Manufacturing procedure
10. Complete Test Methods done on the finished product
11. Unattached legible, comprehensive and indelible specimen of all labeling materials per pack
size [e.g. outer, immediate, package insert (if applicable)]
12. Finished commercial product per pack size
13. Bioefficacy study
 Abstract
 Introduction
 Materials and Methods
 Results and Discussion
 Summary and Conclusion
 References
14. Safety Data Sheet of Finished product
15. Toxicity study
