AFFIDAVIT OF UNDERTAKING

I, CRESENCIO A. SOSA, Filipino, of legal age, married to Corrine O. Sosa, with

residence address at Block 8 Lot 13, Camella Homes Subdivision, Candau-Ay,
Dumaguete City, after having been subscribed to an oath, depose and say:

1. That I am applying for a Business Permit with the City Government of Dumaguete;

2. That my nominated business address is the same as my home address as stated above;

3. That I assure and take full responsibility to save the CITY PLANNING &
DEVELOPMENT OFFICE harmless and free from any legal implication in connection
with my application for Business Permit.
