2. The regulation of body temperature is one of the most critical functions of the nervous
system. In studies of thermoregulation, it is common to divide the body into two
compartments: (1) the external shell, which includes the skin and largely fluctuates in
temperature along with the environment; and (2) the internal core, which includes the
CNS and viscera and has a relatively stable temperature (Jessen, 1985; Romanovsky et
al., 2009).
The core temperature defended by the thermoregulatory system (the balance point or set
point) is not a fixed value but fluctuates in response to internal and external factors. Many
of these factors are unrelated to temperature and instead reflect interactions with other
physiologic systems. One example is fever, which is the controlled increase of body
temperature that occurs most commonly in response to an infection (Figure 1). Fever is
triggered by bacterial lipids and other molecules (pyrogens) that directly or indirectly
induce the production of prostaglandin E2 (PGE2) by endothelial cells lining the POA
(Evans et al., 2015).

Fever or pyrexia, is the result of the thermoregulatory mechanisms' response to an


elevated set point. The set point is raised when acted on by endogenous pyrogen, a
substance liberated by leukocytes when they interact with exogenous pyrogens such as
microorganisms, nonmicrobial antigens, or certain steroid hormones. When endogenous
pyrogen is liberated into the bloodstream, it interacts with the preoptic regions of the
anterior hypothalamus and raises the thermoregulatory set point to a variable degree, but
usually not greater than 41.1°C (106 F). Endogenous pyrogen causes increased firing of
hypothalamic, thermosensitive neurons, resulting in the augmentation of heat
conservation and production mechanisms, with resultant fever .

Sustained or continued fever, characterized by temperature elevation with little (less than
1°C) diurnal fluctuation, is seen as pneumococcal pneumonia and typhoid fever caused
by infection or rat bite fever due to Spirillum minus.

How the body can maintain body temperature?

To achieve a constant temperature, the human body keeps a careful balance between heat
production and heat loss. Heat production is easy - most of the energy we take in with our
food is transformed into heat by our muscles and internal organs. When we get too cold,
our muscles start to shiver, thus producing more heat, and our capillaries contract,
keeping the heat inside.

Temperature is usually controlled by the part of the brain called the hypothalamus. The
hypothalamus is like a thermostat for the body. It maintains normal temperature through
heating mechanisms, such as shivering and metabolism, and cooling mechanisms, such as
sweating and dilating (opening) blood vessels close to the skin
