Graduation Project: 
Service Learning Experience (SLE) Proposal Form 
 
 
DIRECTIONS:​ ​Answer the questions below​ ​before ​developing a ​detailed SLE plan​ on the following page. 
 
 
1. What is your Graduation Project ​topic ​(from your Research Paper)? ​Note: ​If you are changing your topic 
or did not complete a Research Paper, STOP and discuss your topic with your teacher before moving on. 
 
My graduation project Topic is Choosing your own education
 
 
2. What is the ​issue​ or problem (that you identified in your Research Paper thesis) that you will work to ​solve 
through your Service Learning Experience (SLE)? 
 
The issue is that students are not getting the opportunities or developing the skills they need. Also, Students
are dropping out or skipping school because lack of interest or not understanding and getting the help they
need.
 
 
3. What will you ​do​ or create through your SLE to address the issue? (See the ​SLE Ideas page​ for example 
activities and products; you may choose to complete a combination from the list or develop your own.) 
 
Make and pass out flyers and make survey questions, to send the head of cms the results of what can change
inside of Garinger High School
 
 
4. (a) Who will you ​help ​through your SLE? Identify the​ people/community​ that you will serve, ​and  
(b) What ​evidence ​could you provide to show the judges that your SLE was ​successful​?

Note:​ A mentor is ​optional​, but working closely with a professional who can guide your service learning

DIRECTIONS: ​Based on your responses above, ​create a ​detailed plan​ for your SLE​. Your plan must reflect: 
 
✓ Chronological thinking (start at the beginning and plan ​each step​ of your project) 
✓ Estimated ​dates for your activities 
○ Note: Your SLE should be complete ​by Monday, May 6​. 
✓ Realistic time estimates for each activity (an email does ​not​ take 1 hour to write) 
✓ Realistic activities (based on your schedule and resources) 
✓ Anticipated materials needed and evidence collected  
✓ A ​minimum​ of 15 hours (total) of work 
 
Important:​ This is a plan. It is ONLY a plan. Your plan is a ​subject to change​. (Your actual SLE hours 
will be logged on a separate sheet.) However, in order to begin your SLE, ​you must first have a plan 
approved by your teacher​. Why? Because any legitimate project begins with a plan. It is your job to 
start with one, reflect on the plan and your work along the way, and make changes if and when 
necessary. Good luck! (Not that you need it.)
