CASE#1

Patient ZYX is a 67-year old male weighing 145 lbs. diagnosed with renal failure. The attending nephrologist requested
for laboratory test to determine its serum creatinine and the result is 3.2 mg/dL. Because of a recurrent infection, he was
prescribed Cefoxitin (Monowel) 1g IV q6.

Dosing of Cefoxitin in adult with reduced renal function is as follows:


CrCl 30-50mL/min: Administer 1-2g q8 or q12
CrCl 10-30mL/min: Administer 1-2g q12 or q24

CASE#2

A hypertensive patient WVU was prescribed Amlodipine (Norvasc) 5mg PO OD by his family doctor. He has a co-worker
who is also hypertensive in maintenance medication of Vasalat 5mg tablet. Upon the recommendation of his co-worker,
he also started taking Vasalt 5mg together with his own maintenance medication.

CASE#3

Patient TSR weighs 156 lbs., a discharge patient s/p appendectomy. Her attending physician ordered the following as
take home medications:
Continue Ceftriaxone (Rocephin) 500mg PO q12
Mefenamic acid (Dolfenal) 500mg PO as needed for pain

CASE#4

Patient QPO is a 16-year old female diagnosed with nosocomial infection. Upon cell culture and sensitivity testing, it was
found out that the infection was caused by Streptococcus pneumoniae. The patient exhibited fever, cough and chills. She
has the following medications to manage her disease:
Paracetamol (Biogesic) 500mg tab PO q6 for fever
Carbocisteine (Solmux) 500mg cap PO q8 for cough

CASE#5

Patient MNL is a 55-year old male with a history of hypertension, well-controlled on Losartan (Lifezar) 50mg tab as his
maintenance medication. He went to a community pharmacy with his prescription for refill. Upon checking the
prescription, it was dated two months ago. When you ask the patient, he told “I was feeling fine during my last check-up
and the doctor said that my blood pressure was good, so I thought I was cured and didn’t need the medication anymore.”
When you asked him, what prompted him to pick-up the medication now, his response was “I haven’t been feeling well
lately and figured my blood pressure was high.” The patient admitted not regularly checking his blood pressure due to
not having a blood pressure monitoring at home.
